Birth Certificate Registration

Note:
i. A child born overseas to Malaysian parents or father (if mother is non-Malaysian) has to be registered at the Embassy within one (1) year from the date of birth.

ii. After the above said period the child can only be registered at the Malaysian Registration Department in Malaysia.
iii. A Malaysian passport can only be applied for the child, after Form W has been issued by the Embassy.

Procedures:
i. Birth certificate issued by the State of Kuwait has to be duly attested by the Ministry of Foreign Affairs of the State of Kuwait.
ii. Duly complete Form D. Form D is only available at the Embassy.

Note:
The parents/parent has to bring the child to the Embassy to complete procedures required in Form D.

iii. Upon completing Form D, Form W (Birth Certificate Registration) will be issued by the Embassy.

iv. Fees:
Form D : RM 20.00
Form W: RM 10.00

v. Documents:
One (1) copy of parents’ passport (regardless if mother is non-Malaysian)
One (1) copy of parents’ Malaysian IC (only applies for Malaysian parents/parent)
Four (4) copies of passport-sized photos of the child (close-up)
One (1) copy of parents’ marriage certificate

Note:
i. Please bring along the original set of the documents mentioned above for verification.
ii. If any of the originals are not available, then attested copies by the Ministry of Foreign Affairs of Malaysia is acceptable.

Malaysian who lost the passport should :
i. Lodge a police report
ii. Refer to the Malaysian Embassy to submit the application for passport replacement by completing all the following requirements

** Identification Card
** Parent’s Identification Card (for applicant below 18 years)
** Application Form IM.42 Pin 1/97
** Questionnaire Form IM42c
** 2 copies of recent photograph with light blue background in the size of 3.5 X 5 cm.
** Copy of lost passport (if any)
** Police Report

About Us :
The construction of the complex of the Embassy of Malaysia in Kuwait started on 10 February 2001 on a 4,000 square metre plot. The complex, located at Plot No.5, Yemen Street, Diplomatic Area, Block 5, Daiya, Kuwait City consists of two separate buildings, namely the Chancery building and the Official Residence of the Ambassador or Rumah Malaysia.

The construction of the Embassy was completed on 28 December 2002 and it was fully operational by 5 January 2003. The design of the complex is a mix of local and Malaysian architecture, to reflect the cordial relations between Malaysia and Kuwait.
