What is HSS NWT Optometrist Licence?

Optometrists are healthcare professionals who provide primary vision care ranging from sight testing and correction to the diagnosis, treatment, and management of vision changes. An optometrist is not a medical doctor. Applicant should have completed two years or more of education in optometry in regular mode (for Diploma) and three years (for Degree) plus one year of internship (CMOC curriculum to be followed for degree courses). Exceptions, such as AIIMS and SNDT College, would be considered if they have done the course after 10+2.

Requirements For Optometrist Registration

The Requirements For Optometrist Registration are given below,
1. Completed application, photograph attached.
2. Certified true copy of optometry degree from a recognized school of optometry in Canada.
3. Photocopy of successful completion of Canadian Standard Assessment in Optometry (CSAO).
4. Certificate of Good Standing from each jurisdiction applicant is/has been licensed.

NOTE: To be eligible to be licensed in the NWT, current unrestricted registration in a province of Canada is required.
5. Three (3) letters of reference, sent directly to the Registrar from the referee. Reference forms supplied with the application package.
6. Up-to-date resume.
7. Photocopy of birth certificate or citizenship document (if Canadian citizen); or valid immigration or work permit (if not a Canadian citizen)

Renew Your Optometrist Licence

What You Need To Provide?
**Completed NWT Licence Renewal Application Form before March 1st of the new calendar year.
**Credit card authorization or cheque or money order made payable to Government of Northwest Territories.

Note:
**Registrar, Professional Licensing will email NWT Licence Renewal Form on January 15th. Licence and other correspondence will be sent to mailing address unless advised otherwise.
**All active registrants will receive renewal notification mid-January and the renewal application from the Office of the Registrar, Professional Licensing, Department of Health and Social Services.
