Application for Replacement of Lost Identity Card

Reason For Application:
Regulation 13 of the National Registration Regulations 1990 (Amendment 2007) states that a person who has lost his/her identity card shall immediately apply for a replacement.

Application Conditions:
1. The applicant is required to go to any NRD counter; or
2. The applicant can apply for replacement of lost identity card online at www.jpn.gov.my using the “MyKad Replacement System” application if no change of photograph or particulars of the Malaysian applicant is involved.

Application Procedure:
The applicant is required to go to any NRD counter. No documents are required but supporting documents may be presented (originals and copies).

Supporting Documents (Originals And Copies):
1. Certification letter or Welfare Card from the Department of Social Welfare, or certification letter from the Department of Orang Asli Development, a village headman or chief, or the district office or chairman of the Village Development and Security Committee (for the poor, needy or disabled persons); or
2. Certification letter from the Department of Social Welfare or the Fire and Rescue Department or a village headman or chief or the district office or chairman of the Village Development and Security Committee (for those impacted by natural disasters); or
3. Certification letter from the prison or rehabilitation centre (for ex-prisoners or centre trainees); or
4. Student card or certification letter from the school principal or the registrar of institution of higher learning [for students from any school, college or institution of higher learning both local and foreign]; or
5. Lost identity cards resulting from burglary, robbery, theft, catastrophe and the like require submission of a police report clearly stating details of the incident and the lost identity card.

Payment/Fee:
Refunds will not be considered for lost identity card applications that have been processed by the Identity Card Division or if the lost identity card is subsequently recovered.

FAQs :
Does the NRD accept other modes of payment other than cash?
Yes. The NRD also accepts electronic payment such as credit card debit card, and MEPS. However, a minimum amount of RM50.00 is acceptable for payment using credit card.

Can I change my address without changing my identity card? If yes, what is the fee incurred?
Yes, you can. A RM2.00 fee will be imposed for updating the new address into the chip. If you wish to change the identity card, the fee is RM10.00.

How long does it take to process the MyKad application?
Applicants who choose to collect their MyKad at the Putrajaya NRD Headquarters may do so within 24 hours. Those who choose to collect it at any NRD branch offices in Peninsular Malaysia will be able to do so within 10 working days.

Any NRD branch offices in Sabah, Sarawak or Labuan will have the card ready for collection within 30 working days. This charter on issue of the MyKad applies only to applications by Malaysian citizens and applications that have no complications.

What is the fee incurred if I lose my identity card?
The replacement fee for a lost identity card depends on the number of times it is lost. Refer to the payment schedule.
