MOH COVID-19 Vaccination Registration

The COVID-19 vaccination programme is being progressively rolled out to all Singaporeans and long-term residents. All persons aged 12 and above, starting with Singapore Citizens, can now register for their vaccination below.

Instructions

** For individuals below 18 years of age, and who are not part of the MOE vaccination exercise, the parent/guardian should register on behalf of the individual, and fill in the contact details as indicated below.

** You will receive an SMS at the mobile number you register below, as soon as there are slots available for booking. If you require any assistance, please call the MOH COVID-19 hotline at 1800-333-9999.

** Due to the strong demand for COVID-19 vaccinations in Singapore and limited supplies, we seek your understanding that there may be limited slots available at your preferred location.

** However, we wish to inform you that there are appointment slots available at other locations where you may wish to consider to book your appointments. Thank you for your patience and understanding.

How to Register?

Steps :
Step 1 : Visit the official website through provided above.
Step 2 : Read the instructions carefully before you are applying.
Step 3 : Fill the following details,

1. Mobile Number. SMS notifications for appointment booking and reminders will be sent to this number
2. I am registering for
** Myself
** My child / ward (not part of MOE vaccination exercise)

3. Patient’s Name
4. Patient’s NRIC/FIN. Please ensure that the NRIC/FIN you provide is accurate

5. Patient’s Date of Birth
Please ensure that the Date of Birth you provide is accurate
Please type in DD MMM YYYY, e.g. 09 Aug 1965

Step 4 : Finally click on “Submit” button to complete your registration.

How to Sign Up?

** COVID-19 vaccination is free for all Singaporeans and long-term residents in Singapore. As vaccine supply is currently limited, vaccinations will be rolled out in phases, starting with groups who are at greater risk and need the vaccine most.

** COVID-19 vaccination for seniors commenced island-wide from 22 February. From 19 May onwards, all persons aged 40 years and above can register for vaccination by clicking on the link provided above.

1. Is the vaccine free?
The vaccine is free for all Singaporeans and long-term residents in Singapore, which includes Singapore Permanent Residents, and Employment Pass holders.

2. Who is eligible for the vaccine?
At the moment, only healthcare, frontline workers and persons from 40 years old and above, and Students aged 12 years old and above

3. I lost my SMS/booking code. How can I retrieve the link?
Please call MOH at 1800 333 9999 for assistance.

4. Can I change my contact information?
If you have changed your mobile number or email address, please call MOH at 1800 333 9999 for assistance.

5. How do I reschedule my appointments?
You may reschedule your appointments using the same booking link in the original SMS.
