Issuing Family Register for the first time:

To obtain a Family Register, the applicant must:
** Submit the original Marriage Certificate and a copy of it.
** Submit the original and a copy of Family Register of the father of the wife.

The Family Register is issued to married couples only.

** Submit the husband’s original Civil Affairs ID card and a copy of it.
** Submit two (2) recent frontal colour photographs, size 4×6 cm. without glasses for both the husband and the father-in-law.
** Fill the specified # (58) form.

Renew Family Register:

The Family Register (Family Book) is renewed in the following cases:
1.To add a dependent (newborn, wife or a divorced re-married wife).

The followings are required in order to add a dependent:
** Provide the original Family Register (Family Book) or the Civil Affairs ID card with respective copies.
** Provide the original Birth Certificate of the dependent and a copy of it.
** Two (2) recent frontal colour photographs, size 4×6 cm.without glasses for the father.
** The specified # (58) form duly filled.

2. To amend or change citizen’s data or data of one of his family members.

3. To relocate one of the family members.

Note:
In cases number 2 and 3, applicant must submit two (2) recent frontal colour photographs, size 4×6 cm. without glasses.

Replace lost or damaged Family Register:

To obtain a replacement for a lost or damaged Family Register, the applicant must submit:
** A copy of the Family Register, if available.
** The original Civil Affairs ID card and a copy of it.
** Two (2) recent frontal colour photographs, size 4×6 cm. without glasses.
** The specified # (58) form duly filled.
