How To Apply For DHA UAE Medical Fitness?

To Apply For DHA UAE Medical Fitness, Kindly follow the below procedure

When you apply for residence/work visa in your country, Go for a medical examination at a certified health center In your home country who will issue a medical examination certificate.Provide a copy of the medical examination certificate Issued by the certified health center and a copy of the passport to the Ministry of Interior and the Ministry of Labour In the UAE, along with the form approved by the Ministry of Interior.

After getting the visa and on reaching UAE:
Go to the centers of Preventive Medicine Departments at the Ministry of Health and fitness centers and health screening of other health authorities, to conduct confirmatory examination in accordance with the Coundl of Ministers Resolution No.(28) for the year 2011, and provide the following documents:
** Ortgtnal passport
** Ortgtnal residence/work vtsa.
** Ortgtnal health certificate tssued by the certified health Centre by GAMCA ,and certified from UAE mission Inyour home country.

When renewing residence/work visa:
The medical examination will be conducted only in UAE through the Preventive Medicine Departments at Ministry of Health and and fittness centers and health screening of other health authorities in accordance with the decision of the Council of Ministers.

You need a health card number (either valid or temporary).
If you have a health card but do not remember the number, please call Al Muhaisna Center at 04-5023939 asking for your number.
If you do not have a health card, please visit e-Health Card site and apply for a temporary number.
The medical fitness service is for age 18 and above.

Enter a health card number or eMF reference number below to enquire about the status of your payment. If you have several requests, it is preferable to search by the health card number. Please note that you can only see one month old transactions

How To Reprint Form?

You can apply for a DHA UAE Medical Fitness online through the DHA website or the DHA Smart App.

Here are the steps on how to apply online through the website:
1. Go to the DHA website (www.dha.gov.ae).
2. Click on the “Medical Fitness” tab in the menu on the left of your screen.
3. Click on the “Apply for Medical Fitness Online” button.
4. You will need to create an account or log in to your existing account.
5. Fill in the application form and upload the required documents.
6. Pay the application fee.
7. Submit your application.

The required documents for the medical fitness application are:
** A copy of your passport.
** A copy of your residence permit.
** A recent passport-sized photograph.
** A medical report from your doctor, stating that you are fit to travel.

The application fee for a DHA UAE Medical Fitness is AED 200. Once you have submitted your application, you will be notified of the outcome within a few days. If your application is successful, you will be able to download your medical fitness certificate from the DHA website.

Here are the steps on how to apply online through the DHA Smart App:
1. Download the DHA Smart App from the App Store or Google Play.
2. Open the app and sign in to your account.
3. Click on the “Medical Fitness” tab.
4. Click on the “Apply for Medical Fitness Online” button.
5. Fill in the application form and upload the required documents.
6. Pay the application fee.
7. Submit your application.

The steps for applying for a DHA UAE Medical Fitness online are the same whether you apply through the website or the app. The only difference is that you will need to download the app if you want to apply through the app.
