BMDC Certificate Registration

Duplicate Registration Certificate :
1. Application in plain paper for duplicate registration certificate addressed to Registrar, B.M. & D.C. stating nature of loss.

Related / Similar Service : MOPA PMIS Bangladesh

2. Advertisement in two separate Daily national newspaper quoting registration number.
3. General diary from police station mentioning nature of loss, registration number, name of registration issuing body etc.

4. Affidavit from the court of 1st class magistrate or by a notary public mentioning as in Sl. no.3.

5. Two copies of recent pass-port size photograph attested on back side by a Registered Physician quoting his/ her Registration number /1st class Gazetted Officer with seal and signature.

6. Two character certificate from two Registered Physician quoting his/ her registration number /1st class Gazetted Officer with seal and signature.

7. Fee for duplicate registration certificate after acceptance of papers to be paid by bank draft / pay order / Cash , payable to Bangladesh Medical and Dental Council.

Provisional Registration :
1. Prescribed form for registration (provisional / full) to be filled up by the applicant in his / her own legible handwriting.
2. M.B.B.S. provisional / original certificate / college testimonial together with a photostate copy.

3. Provisional registration fee only to be paid by bank draft /pay order / Cash , payable to Bangladesh Medical and Dental Council.
4. Declaration form of Code of Medical Ethics to be signed.

Full Registration- M.B.B.S, BDS :
1. M.B.B.S. original certificate together with photostat copy.
2. Original Internship training certificate together with photostat copy.
3. Provisional registration certificate to be surrendered.

4. Full registration fee to be paid by bank draft / pay order / Cash, payable to Bangladesh Medical and Dental Council.
5. Two copies of recent passport size photograph attested on back side by the Principal of the respective college.

For Full Registration (B.D.S) :
1. Prescribed form for full registration to be filled up by the applicant in his / her own legible handwriting.
2. Original Internship training certificate together with photostat copy.
3. Original B.D.S. certificate together with Photostat copy both side.

4. Two copies of recent passport size photograph attested on backside by the Principal of the respective college.
5. B.D.S. full registration fee to be paid by bank draft / pay order / Cash , payable to Bangladesh Medical and Dental Council.

Renewal Registration- MBBS, BDS, MATS :
1. Renewal Form (Available in this office & also in Website) to be duly filled up.
2. Fees. (See the chart of fees fixed by Council)

3. Original BM&DC Registration Certificate. If Registration Certificate laminated, a photocopy along with the laminated certificate.

Requirements for Eligibility Certificate :
1. Original SSC/ Equivalent Certificate & Mark sheet with Photocopy.
2. Original HSC/ Equivalent Certificate & Mark sheet with Photocopy
3. Two copies of recent passport size photograph attested on back side by Registered Medical Practitioners or by 1st Gazetted Officer.

4. Birth Registration Certificate with Photocopy
5. Equivalence Certificate from D.G. Health for ‘O’ & ‘A’ Level student.

6. Eligibility Certificate fee Tk 1000/= (0ne thousand) only to be paid by bank draft / pay order/ Cash, payable to Bangladesh Medical and Dental Council.
