Apply for a Blue Badge :
When visiting a One Stop Shop to submit your application you must ensure you have all of the following with you :
** The completed application form.
** 1 photograph – passport style

** Proof of residency
** Proof of ID – Details of acceptable documents / evidence is provided in the guidance notes of the application form
** Proof of eligibility
** Payment of £10.00 by Cheque or credit ordebit card only

When visiting a One Stop Shop to submit your application you must ensure you have all of the following with you :
** The completed application form.
** Evidence of your business address.
** A photocopy of the tax discs for any vehicles registered under the Disabled Passenger Vehicle (DPV) class
** Payment of £10.00 by Cheque or credit or debit card only.

Renew a Blue Badge :
** You can renew your Blue Badge permit up to two months before it expires.
** You are advised to apply at least one month before the expiry date.

Blue Badge Renewal Application :
** You can use this form to apply to renew a Blue Badge if it is due to expire in the next 2 months or if it has expired within the last 2 months.
** If you badge expired more than 2 months ago or you do not currently have a Blue Badge you will need to complete a new Blue Badge application form.

** If you are applying for a replacement Blue Badge then please use the designated Blue Badge Replacement Form if your badge has been lost, stolen or damaged.
** Please complete all relevant sections of the application form and supply the appropriate documents to confirm your address, identity and evidence of eligibility.

** You may find the accompanying guidance notes helpful.
** The local authority may refuse to issue a badge if you do not provide adequate evidence that you meet the eligibility criteria.
** You will need to visit a One Stop Shop at least once so that we can verify your identity and confirm that your photo is a true likeness of you.
** If you are completing the form on behalf of an applicant who is under 16, or someone who is unable to complete the form themselves, please provide their details in the appropriate sections and sign the form on their behalf.

Replace a Blue Badge :
Eligibility :
** We may ask for your permission to contact your doctor for additional information about your disability to determine your eligibility.
** Only the council can decide whether or not you are eligible for a Blue Badge.
** If your medical condition or mobility improves, resulting in you no longer being eligible then you must return your Blue Badge to the council immediately.
