Register Birth & Death :

Register Birth :
Overview :
** All births in Singapore have to be registered at one of the Birth Registration Centres.
** Upon registration, the newborn will be issued a Birth Certificate.

Eligibility :
** Parents of the newborn may register the birth.
** A proxy may also register on the behalf of the parents, provided the proxy has a letter of authorisation by the parents.

When to Register Birth :
Birth Registration :
** Births should be registered within 14 days from the date of birth, including Sundays and public holidays.

Delayed Registration :
** Delayed registration refers to births registered after 14 days and within 42 days.
** Delayed registration is strongly discouraged.

Late Registration :
** If registration is done after 42 days, a letter of explanation stating the reason for late registration must be submitted for the Registrar/Registrar General’s approval.
** Late registration cases can only be registered at ICA.

Documents Required :
For ALL :
** Notification of Live Birth issued by the hospital*
** Identity Cards of both parents
** Original marriage certificate of parents, with English translation (if applicable)
** Parents of children not born in hospitals can obtain the Notification of Live Birth from the doctors/midwife/ ambulance staff who delivered the baby.

Additional Documents Required for Foreigners :
** Passports of parents
** Entry Permits of parents
** Disembarkation/Embarkation card of parents

Additional Documents Required for Registration by Proxy :
** A letter of authorisation from the parents of the child

Registration Venue :
Births can be registered at any of the following hospitals where the child was born:
** Gleneagles Hospital
** KK Women’s & Children’s Hospital
** Mount Alvernia Hospital
** Mount Elizabeth Hospital
** Mount Elizabeth Novena Hospital
** National University Hospital
** Raffles Hospital
** Parkway East Hospital
** Singapore General Hospital
** Thomson Medical Centre

Or at the Registry of Births & Deaths at the following address:

Operating Hours:
** Monday to Friday – 8.00 a.m. to 4.30 p.m.
** Saturday – 8.00 a.m. to 12.30 noon
** For births in hospitals not listed as a registration centre, please register at the Registry of Births & Deaths, Citizen Services Centre.

Procedure :
** The following steps apply for registrations at Citizen Services Centre, ICA and may differ from the hospital registration procedures.
** Please check with the respective Birth Registration Centre if registering through a hospital.
** Ensure you have the Child’s name*
** Proceed to Information Counter or any of the self-ticketing kiosks at the Registry of Births & Deaths to obtain a Q-number and Birth Report form

** Complete the Birth Report form at the form filling counter
** Wait to be called
** The Registration Officer will verify the documents produced and print out a sample of the birth certificate.
** Verify the particulars of the child
** Sign on the Birth Certificate that the Registration Officer will print out
** Wait for the Birth Certificate to be laminated and issued
** Ethnic characters of the child’s name in Chinese/Jawi/Tamil (optional) may be included in the Birth Certificate but the parents/proxy must ensure the accuracy of the characters.
** For Chinese children who have been given a name in full Hanyu Pinyin version, the Chinese characters of the name must be reflected in the birth certificate.
** From 2 January 2010, parents will be required to declare their child’s race during birth registration.

Fees : S$18
** Some hospitals may charge an administrative fee on top of the $18.
** Please make payment by NETS (for payment at ICA).

Processing Time : On the spot

Peak Periods to Avoid at ICA Building :
** Mondays, Fridays, Saturdays, eve and the day after public holidays
** After 11.00 am on weekdays and Saturdays

Statistics on Births in Singapore :
** The Registry of Births and Deaths publishes an Annual Report on the Birth and Death Statistics in Singapore.
** You may purchase a copy of the report at Counter 32, 3rd Storey of ICA Building.

Register Death :
Documents Required :
** Certificate of Cause of Death issued by doctors or authorised medical practitioners (See Procedure on how to obtain this certificate)
** Identification documents of the deceased (e.g. IC and Passport) for cancellation
** Registrant’s identification documents (e.g. IC/Passport)

Procedure :
** For Deaths Occurring at Government/Government-restructured Hospitals

** KK Women’s & Children’s Hospital
** Singapore General Hospital
** Tan Tock Seng Hospital

** Citizen Services Centre, 3rd Storey, ICA Building

How to Register?
** Produce the Certificate of Cause of Death issued by a doctor at the hospital and the necessary supporting documents to the Registration Officer
** Verify the particulars in the sample Death Certificate printed by the Registration Officer
** Sign the Death Certificate printed by the Registration Officer upon verification
** Collect the Death Certificate

Fees : Free of charge

** On the spot at Citizen Services Centre, ICA Building
