ICA Apply for a Death Extract

You may apply for a Death Extract using eXtracts Online. An extract is a certified true copy of Death Certificate. ICA’s Registry of Births and Deaths maintains all the death records, which were registered in Singapore since 1872.

To apply for a Death Extract, you must provide the following information :
** the Name, Sex and Date of Death contain in the Death Certificate; AND
** Singapore Death Registration Number; OR If the Deceased died within the last 5 years, the Deceased’s NRIC / Birth Certificate Number.

You will need an e-mail account and any one of the following for submitting an application using eXtracts Online
** VISA Card; or
** MasterCard Credit/Debit Card; or
** American Express Card (AMEX); or
** Internet Banking (Citibank, DBS/POSB or UOB)

Eligibility

A death extract is a certified true copy of the death certificate. It replaces a lost or damaged death certificate. Anyone can apply for a certified death extract. Law firms must apply using eService.

Documents Required

You will need the following information about the death certificate holder to apply for the death extract. Please have it ready before you apply :
** Name
** Gender
** Date of death

** Singapore death registration number or, if the deceased died within the last five years, Singapore Identity Card (IC) or death certificate number. If you cannot provide the death registration, the IC or death certificate number, you will need to pay a search fee.

You will need to tell us the approximate period during which you believe the deceased passed away. We will only check records that fall within the nominated search period.

Procedure

Online :
You can apply for a death extract online using eService.
In person :
To apply in person, you will need to provide your :
** Identity Card (for Singapore citizens and permanent residents)
** Passport or Long-Term Pass card (for foreign visitors).

You will need to visit :

Citizen Services Centre
ICA Building
10 Kallang Road, Level 3 (next to Lavender MRT station)
Singapore 208718
Operating hours are 8.00 am to 4.30 pm on Monday to Friday, and 8.00 am to 12.30 pm on Saturday.

Longer-than-average waiting times are to be expected on the following peak times :
** Monday, Friday, Saturday
** School holidays
** The day before or after a public holiday. Please remember to obtain a receipt, as you will need it to collect the death extract.

Fees

Upon approval, the following fees are payable :
** S$40 for an extract
** S$40 for a one-month search period
** S$40 for every additional month or part thereof.

The search fee is in addition to the extract fee that needs to be paid after the record is found. It is non-refundable.

For online applications, payment can be made using either :
** Visa or MasterCard credit/debit card,
** American Express (AMEX) credit card,

** Internet direct debit (DBS/POSB, OCBC, UOB, Standard Chartered Bank and Citibank internet banking accounts in Singapore). For applications in person, payment can be made using NETS or CashCard.

Processing Time

** The death extract should be ready to collect in four working days (excluding the day of submission and collection).

** If the processing time is longer, we will contact you when the extract is ready for collection.
** For a death record search, we will contact you once the search results are available.

Collection

You can collect the certified death extract :
** Via post (please note, ICA will not be responsible for any damage caused during the mailing process)

** In person or by proxy at :
Registry of Births & Deaths
Citizen Services Centre
ICA Building
10 Kallang Road, Level 3 (next to Lavender MRT station)
Singapore 208718

You will need to produce the following documents when collecting the death extract :
** Identity card (for Singapore citizens and permanent residents)
** Passport or Long-Term Pass card (for foreign visitors)
** The official payment receipt or an application reference number.

Other than the above documents, a proxy will also need to produce a letter of authorisation that mentions his/her name and identification number
