ABN License Verification

The Alabama Board of Nursing obtains primary source verification of education prior to issuance of licenses. For advanced practice nurses, primary source verification of certification occurs prior to issuance of approval.

License Lookup on the Board’s web site is not considered primary source verification. An update is run only once a day. However, subscribers to the group online license verification system can count on the system for primary source verification. If requesting a license verification, please allow 3 to 5 days for your verification to be PROCESSED and MAILED.

Note:
The online applications will not currently work with Firefox. Please use Internet Explorer or Chrome. Also, make sure scripting is enabled.

Requirements :
To Request An Official License Verification:
You will need the name of the licensed nurse, the license number, and the last 4 digits of the social security number. You can request that the official verification be sent to another Board of Nursing, or any other location. Requests must be made online at HERE and payment occurs with a credit card or debit card. Personal checks are not accepted for license verifications.

Name and Address Change:
If you make a name change it will not be processed until we receive appropriate documentation in our office. You may FAX to (334) 293-5201 attn: Carol or email to carol.smith@abn.alabama.gov : a copy of marriage certificate, divorce decree (which states the name change in the decree),or probate court records reflecting a legal name change.

For proper identification, please include your nursing license number or social security number on all documents faxed, emailed, or mailed to us.

Note:
If a Duplicate License card is needed you must pay the $25.00 fee under the Request for Duplicate License in order to receive a new card.

Nurses may now make address changes online. Please keep your most current mailing address on file. After making your change, please allow our system to update the next business day after 9am.

Reinstatement :
If you previously held an Alabama LPN or RN license, and your license is presently in a lapsed status, you may apply for the reinstatement of your lapsed license.

An RN license that is reinstated between now and 08/31/2016 will receive an active license with an expiration date of 12/31/2016.

An LPN license that is reinstated between now and 08/31/2015 will receive an active license with an expiration date of 12/31/2015.

Reinstatement of a lapsed license requires:
** A completed application for reinstatement
** Payment of the reinstatement fee
** You must have 24 contact hours to reinstate. All required contact hours may be obtained from the following activities: workshops, seminars, classroom, web casts, internet, intranet, home study, continuing education journals, and pod casts. Contact hours should be no older than 24 months from the date we receive your application. All CE certificates must be mailed or faxed to our office. CE certificates can be sent via facsimile to 334-293-5201, Attn: Javonda Kennedy.
** Proof of United States Citizenship or Lawful Presence: Alabama immigration law requires that you prove U.S. citizenship or that you are in the United States lawfully. Please complete the appropriate checklist and attach a copy of your document to the checklist and submit to the Board of Nursing as directed on the checklist form. We are required to only capture this information one time for U. S. citizens and non-citizens with a permanent resident card. Others will have to supply information at each license renewal.

    I, Bertha M. Gibson graduated from a LPN School, Cobb School of Nursing, in 1989 and the school is closed. My employer needs verification that I am graduated from the School, Cobb School of Nursing, LPN, which was in Phenx City, Alabama. They need an official transcript or a copy of my Diploma. If you can help me please let me know.
