** A copy of the certificate from the psychiatric clinic ward person;
** A copy of the certificate from the STI clinic ward person;
** Copy of the certificate with the tuberculosis hospital ward person;
** Copy of the certificate with drug treatment clinic ward person;
** A copy of the certificate from the body of social protection (of the person transferred under guardianship, the guardian);
** A copy of the certificate from the residence showing all prescription (f-17) (of the person transferred under guardianship, the guardian);
** A copy of the survey apartments ward person issued with the district health department;
** A copy of the plan of the house / apartment (of the person transferred under guardianship, the guardian);
** A copy of the death certificate of the parent entity transferred to custody;
** A copy of the passport of family members guardian;
** A copy of the notarized consent for the issue of guardianship family guardian.
