Mauritius Passport:
Who is eligible for Mauritian passport?:
All Citizens of Mauritius, either by Birth, Descent, Adoption, Registration or Naturalization, are eligible for a Mauritius Passport, provided that they satisfy all conditions prescribed by law.

How to apply ?:
Application Form:
You can collect the Passport Application Form, either at:
** The Enquiry Desk of the Passport & Immigration Main Office, Sterling House, 9-11, Lislet Geoffroy Street, Port Louis, or
** Any Police Station in Mauritius, or
** The nearest Mauritian Embassy/Consulate Office, (in case you are abroad), or
** Download the Form click here ; For Editable Version – click here

Completing the Form:
The Application Form comes with clear and detailed instructions on the front page.
Please follow them carefully and provide precise information, as any error will delay your application.
Complete all sections of the Form which are relevant to you in CAPITAL LETTERS with a BLACK ballpoint pen, well within the white boxed areas.

Please note that:
Consent at Paragraph (4A):
** Is required in case applicant is a minor (under 18 years).
** Is to be signed by both parents (where applicable), in the presence of a Police Officer or the Passport & immigration Officer.
** If parent(s) is/are abroad, consent may be given in the presence of either a Police Officer / an Official of Embassy / a Notary or a Solicitor, from the country of residence.

Specimen Signature at Paragraph (7):
It should be done with a bold black pen of size 0.5 mm – 0.6 mm, well inside the box, as in specimen hereunder.

Police Certificate at Paragraph (8):
** Please refer to Note 5 on the Application Form.
** If applicant is abroad Police Certificate is not required, however, applicant will have to show proof of residence abroad (i.e. valid visa/residence permit).

Declaration of Applicant at Paragraph (9):
** The applicant, if aged 18 years & above needs to sign the Application Form at Paragraph (9).
** In case of minor (under 18 years), one parent need to sign the declaration.

Documents to be Submitted:
You need to submit a duplicate copy of all relevant documents mentioned below (as applicable) together with your Application Form duly filled.

Originals of all documents need to be produced for verification.

In all cases:
** Birth Certificate.
** National Identity Card (if applicable).
** Certificate of Foreign Citizenship (if applicable).
** Certificate of Mauritian Citizenship in case applicant acquired nationality through Registration / Naturalization / Adoption (if applicable).
** Two (2) recent identical photos (passport size) taken against a light background. For more details regarding quality of photo,click here.
** Previous passport or Certificate from Police in case of loss passport (if applicable).
** + Additional documents as applicable (see below).

Minor:
** Above-mentioned documents +
** Birth certificate of parent/s.
** National Identity Card of parents/guardians.
** Judge’s Order (where applicable).

Married/Divorced person applying on Spouse/Joint Name:
** Above-mentioned documents +
** Marriage certificate.
** Divorced decree, in case: (i) applicant intends to keep her ex-spouse’s name (if eligible), or (ii) for change of name where applicant is holder of a passport on her ex-spouse’s name.

Note:
Married woman applying for a passport to be issued on ‘Joint Name’, the name on the passport would be written as follows:- Maiden Name followed by Spouse Name.

  1. I SEND MY DAUGHTER PASSPORT THAT HAS EXPIRED. HER FATHER COULD NOT APPLY FOR A NEW PASSPORT. IN THIS CASE WHAT SHOULD I DO? AND WE ARE RESIDING IN ITALY. SHE GOT 16 ON OCTOBER 2015.
