Creating Your Student Dossier

Below you will find the documents you would require for creating a Dossier:
** A filled out copy of application form No. 401.

** Certificate of registration at the university where the student is studying, including the year of his/her enrollment at the university and their exact study program.(This document should also consist of a transcript grades for every year of study as well as the certificate of the last obtained diploma/degree at the establishment)

** 2 photocopies of all the filled out pages of the applicants passport, including the date of last departure from Iran.
** Abiding by the regulations of Public Service (i.e. holding a Conscription Termination card, document of exemption of 0r deferral due to full-time studies)

** (In applicable) A photocopy of the spouses passport as well as children, and the respective residency documents.
** A certificate approving a legitimate departure from Iran, (in case the date of first departure from Iran has not been indicated in the applicants passport)
** Photocopy of the applicants card and his/her University Student ID card.

** 2 photocopies of the applicants entire Iranian Birth Certificate (Shenasname).
** (In applicable) 2 photocopies of the applicants spouse and childrens Iranian Birth Certificates (Shenasname) is also necessary.

** Two 3x4cm ID photos taken within the past six month, (Including a full face, front view, white background, open eyes, without a hat, glasses 0r neck scarves, with the exception of headscarves(hijab for women)). [The individuals personal details must be inscribed on the back of every piece of photo]

** (In case the student is a Scholar) A photocopy of their recommendation letters and approval letters.

Required documents for those studying Medicine and related studies:
** A filled out copy of application form No. 401.
** Certificate of registration at the university where the student is studying, including the year of his/her enrollment at the university and their exact study program.(This document should also consist of a transcript grades for every year of study as well as the certificate of the last obtained diploma/degree at the establishment)

** 2 photocopies of all the filled out pages of the applicants passport, including page with the date of last departure from Iran.
** Abiding by the regulations of Military Service (i.e. holding a Conscription Termination card, document of exemption of 0r deferral due to full-time studies)

** (If applicable ) A photocopy of the spouses passport as well as children (if any), and the respective residency documents.
** A certificate approving a legitimate departure from Iran, (in case the date of first departure from Iran has not been indicated in the applicants passport)

** Photocopy of the applicants Student residence permit* card and his/her University Student ID card.
** 2 photocopies of the applicants entire Iranian Birth Certificate (Shenasname).
** (In case of being married), also 2 photocopies of the applicants spouse and possible childrens Iranian Birth Certificates (Shenasname) is also necessary.

** (In case the student is a Scholar) A photocopy of their recommendation letters 0r approval letters from the Ministry of Health and Medical Education.

PhD Students and those at the level of specializing, who dont possess a student (academic) residence permit, are obliged to provide the necessary pledges of returning to the country. To accomplish this, the student should have their parent 0r legal representative, contact the student affairs department of the Ministry of Health and Medical Education.

Note:
The documents submitted by student will be transferred to the respective ministries, via the MFA, and upon approval, the student will be entitled to the benefits of student facilities thereby provided.
