Registration Of Birth Certificate (Regular And Timely) :
REQUIREMENTS :
** Accomplished Certificate of Live Birth form
** Photocopy of Parents’ Marriage Certificate
** For Illegitimate Child:
Notarized Affidavit to Use the Surname of the Father if the child was acknowledged

PROCESS:

PROCESS :
** The City Civil Registry Department Receiving clerk will ask the client to secure a Certificate of No Record (CNR)
** The client will go to the City Treasurer’s Office cashier to pay for the CNR (P50.00)
** After getting a CNR, the registrant will submit all the necessary requirements to the clerk
** The receiving clerk will then issue the client a Certificate of Live Birth form
** After completely filling it out, the clerk will issue an order of payment to the client
** The client needs to pay the Late Registration fee (P150.00) to the City Treasurer’s Office cashier
** Once paid, the clerk will give the applicant a document indicating the day of release of the record

How to Get a Death Certificate

A Death Certificate is an official document indicating a deceased person’s record including the date of birth and date of death

REQUIREMENTS :
Accomplished Certificate of Death form
If applicable, Autopsy report

PROCESS :
** Fill out the Certificate of Death form and submit it to the City Civil Registry Department together with other requirements
** The receiving clerk will evaluate if the documents are complete and issue an order of payment
** The client needs to pay the corresponding fee (Registration Fee – P50.00) at the City Treasurer’s Office cashier
** Upon receiving the official receipt, the clerk will release the document to the client

Additional Simplified Procedure

To get a birth certificate in Quezon City, you can follow these steps:
Bring the following requirements:
** Original and photocopy of valid ID of the applicant
** Original and photocopy of birth certificate (if available)
** Application form (available at the CCRD office)
Payment of the certificate fee:
P100 for a regular birth certificate
P150 for a late registration of birth certificate

Fill out the application form and submit it to the CCRD office. Pay the certificate fee. Wait for the release of your birth certificate. The processing time for the release of a birth certificate is 15 working days.

You can also apply for a birth certificate online through the Quezon City government’s website.
Here are the steps involved:
** Go to the Quezon City government’s website: https://quezoncity.gov.ph/
** Click on the “City Civil Registry Department” tab.
** Click on the “Apply for Birth Certificate Online” button.
** Fill out the online application form.
** Upload the required documents.
** Pay the certificate fee using a credit card or debit card.
** Submit your application.

The processing time for the release of a birth certificate through the online application is 30 working days.
