Application, renewal and correction of passports for citizens.

All applications must be made in person, without exception.

Requirements:
Application:
** Duly filled-in application form.
** One colour passport-size photo, signed by the Applicant on the back. (Not required at Windhoek Office)
** Your current passport with good quality certified photocopy of the personal page of your current passport. (If available)
** Previous Applicants: Old Passport, or Police Affidavit if lost/stolen.
** Namibian Birth Certificate (if applicable was born in Namibia).
** Namibian Identity Document (ID).
** Namibian Citizenship Certificate (if applicant was born outside Namibia).
** Married Women: Certified copy of Marriage Certificate (Married women who have not updated their ID to husband’s surname and want his surname on their passport)
** Divorced Women: Certified copy of Divorce Certificate (Divorced women wishing to revert back to their maiden surname)
** Minors (under 16): Original parent’s identification. Any child over 13 must be present for signature.

Corrections:
** Passport
** Birth Certificate or ID showing correct information

Relevant good quality certified photocopies of the above documents should be submitted with your application.

If your passport is lost or stolen, it must be reported to your local Police. You should obtain a document from the Police to say that the passport has been reported missing, and submit this document when you apply for a new passport.
