Step 1: Go to the link https://mycovidrecord.health.nz/
Step 2: Click on Log in button


Step 3: In the ‘Passes and certificates’ section, select ‘Request pass or certificate’.


Step 4: The ‘Select who this pass or certificate is for’ will open. Select either ‘Myself’ or ‘Someone else’.
Step 5: The ‘Select a pass or certificate’ option will appear. Select ‘My Vaccine Pass’ or ‘International Travel Vaccination Certificate.’
Step 6: You will then be prompted to enter details including the full name and date of birth of the person who the pass is for and their NHI number.
Step 7: You should receive your My Vaccine Pass by email within 24 hours.

How to sign up for My Covid Record?

1. Anyone in New Zealand aged 12 or over can sign up for My Covid Record.
2. First, you will need to create a My Health Account, which allows you to access your health information online. You will need your own email address to create an account.


3. You will need to provide one of the following documents so we can confirm your identity.
A New Zealand:
** driver licence
** passport
** birth certificate (from any year)
** or citizenship certificate.

How To get proof of your vaccinations in Other ways?

Other ways to get proof of your vaccinations are given below,

1. Over the Phone
If you cannot access My Covid Record, or do not have a unique email address or valid ID, you can call the Ministry of Health to request a physical copy. It will help to have your NHI number ready.
Phone: 0800 28 29 26, select option 4 (8am to 8pm, 7days a week)

2. In person at a pharmacy
Pharmacies that are providing COVID-19 vaccinations can also help you request and print your My Vaccine Pass, My Vaccine Record or International Travel Vaccination Certificate for free. Confirm in advance that they offer this service, and bring your NHI number if you can when you visit.
You do not need to have ID or an email address to get your pass or certificate from a pharmacy. You will be asked some personal details so you can be found in the system.

3. For a group of people in your care
For a group smaller than 10 people:
To request My Vaccine Passes for groups smaller than 10 people, please encourage and help each individual to get their pass for themselves.

For a group of 10 or more people:
You can only apply for a My Vaccine Pass on behalf of a group if your organisation has a formal and actual care or charge of the people who need help getting their My Vaccine Pass. For example, assisted-living residents at a rest home, or disabled people who require care and support.

You can request My Vaccine Passes for a group of 10 or more people if you have:
** Authority to request each group member’s vaccination record, and can provide evidence of this authority to the Ministry of Health on request
** A record of each group member’s NHI number, and have authority from each group member to share this with the Ministry of Health
** Authority from each group member to share personally identifiable information (name, NHI, date of birth, address) with the Ministry of Health.

4. Find your National Health Index (NHI) number
A National Health Index (NHI) number is a unique number that is given to each person using health and disability support services in New Zealand.
You can find your NHI number on:
** A prescription or prescription receipt, a prescription medicine label
** A hospital letter
** An x-ray or test result
** Your patient portal (Manage My Health).
