What is EU Digital COVID Certificate Self Service Portal?

Welcome to the Digital COVID Certificate Self Service Portal. You can use this portal to manage a number of activities in relation to your Certificate of Vaccination or Certificate of Recovery without needing to call the Digital COVID Certificate call centre. For certificates based on negative RT-PCR tests, a Digital COVID Certificate will be provided by the private RT-PCR testing service. Certificates based on a RT-PCR are valid for 72 hours after the test was taken.

How To Request For COVID-19 Recovery Certificate?

You can use this form to request a certificate (known as a certificate of recovery) to prove that you have recovered from COVID-19 in the last 6 months.

You can request a certificate of recovery if you have had a positive COVID-19 test (RT-PCR or ‘NAAT’) more than 11 days ago. Your certificate will remain valid for 180 days after the day you tested positive. Once you fill out the details below, you will get your certificate by email within 5 days.

Request For Non-EU Country Vaccination EU Digital COVID Certificate

I do not have an EU Digital Covid Certificate. I wish to apply for an EU Digital COVID Certificate for Covid vaccinations received outside the EU. I hold a valid Irish passport and documentation confirming that I have been fully vaccinated in a non-EU country/state. Please Note: Only vaccines authorised for use in Ireland can be recognized for the purpose of this service.

Request an EU Digital COVID Certificate for vaccinations received in a Non-EU Country. You can use this form to register details of vaccinations you received in Non-EU countries. In order to apply for the EU Digital COVID Certificate you will need to:

1. Have received a vaccination type which is approved for use in Ireland. Those vaccines are:
** Moderna
** Oxford-AstraZeneca (Vaxevria/Covishield)
** Johnson & Johnson/Janssen
** Pfizer-BioNtech
** SinoPharm BIBP
** CoronaVac/Sinovac

2. Have proof of vaccination from the country that administered your vaccination.

3. Hold a valid Irish passport and be an adult, that is you have reached your 18th birthday on the day you make your application. Once you fill out the details below, we will review your request and, if eligible, you will receive an EU Digital COVID Certificate by email.
