Paying for Hospitalization of Nursing & Mentally Frail Patients :

The process includes the following steps (details below) :
** Initial contact with the Health Bureau
** Initial submission of documents

** Meeting between the family members and a nurse and social worker from the Health Bureau
** Classification of the candidate for hospitalization
** Selection of the nursing institution
** Calculation of the family copay for the hospitalization
** Commitment of the family to make payment
** Transfer to hospitalization
** 2. Initial submission of documents to the Health Bureau

Required Docyuments :
** Request for institutional arrangement form, filled and signed
** A photocopy of the ID card of the candidate for hospitalization, including the address stub.
** Medical-nursing-functional information form, updated to the last month, filled by a doctor and nurse, based on an examination of the candidate for hospitalization (Click here to download the form)
** Appealing to the head of the geriatrics department

Method of Payment :
** Standing order – the patient and all of the family members liable for payment sign a standing order prior to the hospitalization. It is recommended that payment be remitted via a standing order, which saves time and avoids late charges of interest and indexing.
** Monthly payment vouchers sent to each of the payers to the residential address given. Every family member must report any change of address or marital status. The bill must be paid no later than the 5th of every month to avoid incurring late charges of interest and indexing. These vouchers can be paid as follows :
** At the Postal Bank (in cash or by cheque)
** At the Health Bureau (in cash or via credit card)
** Via the Ministry of Health website (via credit card)

Re-classification of the candidate :
** Initiated by the family : the family may contact the Health Bureau to request re-classification of the candidate, based on a change in the state of the candidate for hospitalization. The documents attesting to the change must be attached.
** Initiated by the Health Office : the health office staff may initiate the re-classification of the candidate, if new information on the matter has come into their possession. Note that if three months have elapsed between the date of performing the medical-nursing assessment of the candidate and the candidate has not yet been hospitalized, then it will be necessary to update the assessment, and in some cases the candidate might need to be re-classified
