Reimbursements for a person retroactively resident status :

** The reimbursement is made directly to the applicant provided the applicant has already paid for the service, or to the hospital if the applicant has yet to pay.

** Reimbursement requests for HMO membership fees, which were paid while the applicant was not a member of an HMO, must be made directly to the National Insurance Institution.
** Reimbursements for a person retroactively granted resident status (the interministerial committee)

Service Description :
Anyone whom the National Insurance Institute has recognized retroactively as an Israeli resident, can submit a request for reimbursement of medical services received during the period that s/he was not a member of an HMO.
The reimbursement is granted only for services purchased during the period of residence, and applies only to health services which are included in the health services basket. Reimbursement is according to the Ministry of Health’s tariff list.

Requirements prior to receiving Service :
Any person recognized retroactively by the National Insurance Institution as a resident of the State of Israel, is eligible for filing a request for reimbursement of funds paid in exchange for medical services received prior to becoming a member of an HMO.

Required Documents :
** The applicant must submit a filled-in, signed Committee Application Form.
** Copies of receipts must be attached, testifying to the payments made for the health services for the requested reimbursement. If the medical service has not yet been paid for, then the applicant must submit a copy of the payment form signed by the applicant, as well as a detailed note specifying the debt amount.
** Attach hospital case summaries and discharge forms, detailing the the treatment provided.
** Attach a detailed list of the medical service provided, at which institution, when and what it cost.
** If the form is filled out for a dependent, then attach a guardianship order. If the application is submitted for more than one dependent, then a separate guardianship order must be attached for each dependent.
** If a application has been submitted to the National Insurance Institution for recognition of residence, then attach the notice from the Institution regarding residence.
** If the application refers to a person who was a soldier prior to receiving the health services referred to in the application, then attach a copy of the IDF discharge certificate.
