How to Apply?

** Application can obtain from any Divisional Secretariat.

** Required fee should pay to the Divisional Secretariat.
** Duly filled application should submit to the Divisional Secretariat.
** If you like to obtain the certificate of birth by post, submit a stamped envelop with the application.
** If the birth is registered a birth certificate will be send and if the birth is not registered a letter to the effect will be send.

Fees

Fees chargeable :
** Where the date of registration or the no. of the entry is given the fee for one copy of the certificate is Rs. 100.00
** Where the date of registration or the no. of the entry is not given and a search of registers not exceeding two years is involved fee for one copy of the certificate is Rs. 200.00

Translate Certificate of Birth

** Offices of the Department where service of sworn translators can be received are given below.

** Translations which can be obtained,
i. Sinhala – English
ii. Tamil – English
iii. English – Sinhala
iv. English – Tamil

Time of receiving applications :
** From 9:00 a.m. – 3:00 p.m.
** Applications may be obtained from the offices where sworn translators are available.

** Documents to be submitted
i. Duly completed application.
ii. Original or certified copy of birth certificate obtained from a Divisional Secretariat of the document to be translated.
iii. Stamped envelope if translations are to be received by post.

** Fee for translations – Rs. 500.00 for one copy payable to the Registrar General’s Department

1. How to obtain a Copies of Birth, death and Marriage Certificates?
Please apply from any Divisional Secretariat in the District where the event occurred and applications can be obtained from any Divisional Secretariat

2. How to Register a Marriage?
Please contact Marriage Registrar in the Division or Additional District Registrar of Divisional Secretariat

3. How to Register a Birth/ Death occurred outside of the Sri Lanka?
Applications can be submitted to the Central Record Room of the Department in Maligawatta, Colombo 10 together with parents Passports, and Child’s as Hospital Record /Birth certificate issued from the relevant country

To register a Death the documents to be submitted to prove the Identity and cause of Death

4. How to Re-register a adopted Child?
Applications can be submitted to the Registrar General’s Head Office at No. 234/A3, Denzil Kobbekaduwa Mawatha, Battaramulla together with parent’s birth certificates and Marriage Certificate, adoption certificate and child’s birth certificate if already registered earlier

5. How to obtain a translation of Registered Birth, Death and Marriage Certificates?
Applications can be forward to Registrar General’s Head Office at No. 234/A3, Denzil Kobbekaduwa Mawatha,Battaramulla, or to the Translators given in the department Web site.

6. How to get amended an entry in a Birth/ Death certificate?
Applications can be submitted to the any Divisional Secretariat within the District where the event occurred. For further information meet Additional district Registrar in the Divisional Secretariat.
