Death Certificate :

An extract or copy of a death certificate is written proof of the fact that an individual has died.

What is it :
A death certificate provides proof of the death of an individual and that this death is recorded with the Registry Office.

How does it work :
** A death certificate is only available in Dutch.
** The international extract is available in other languages. A multiple-language extract comes in Dutch, English, French, German, Spanish, Greek, Italian, Portuguese, Turkish and Serbo-Croatian. If a copy of a document is not available in other languages or if a country does not accept it, then you need to have the copy translated by a sworn translator.
** If the certificate was drawn up more than 100 years ago, then you need to apply at The Hague Municipal Archives.

What are the conditions :
You may only apply for an international extract or a copy of a death certificate in The Hague if the individual died in The Hague.

Do you want to apply for a copy of a foreign death certificate- This is only possible if you have had this certificate registered with the Afdeling Landelijke Taken.

In all cases, you will need to prove a legitimate interest. This means that you may only submit an application :
** as an authorised representative
** as a blood relative (parent or child)
** as a beneficiary
** as a registered partner or spouse
** as a legal representative.

You can also submit this application if you need the document for a legal procedure or to register a partnership or marriage.

What does it cost :
A death certificate costs € 12.50 (rate for 2014) excluding postage costs for a request in writing. You will receive an invoice for this amount.

How do I apply/What do I need to do :
You can submit your application in 3 ways :
Online :
** For this you need DigiD. You can only apply online for certificates with a DigiD if you are registered with a Dutch municipality.
** You will receive the certificate after 3 working days.

In person :
To do this you will need a valid proof of identity. Have you been authorised to apply for an extract or copy on someone else’s behalf- Take (a copy of) this person’s proof of identity and the signed authorisation form which authorises you to arrange this.
** Certificates as of 1986 are available immediately.
** You will receive certificates from before 1986 after 5 working days.
** With an authorisation you will receive the certificate after 10 working days.

** For this option you need (a copy of) your valid proof of identity and the application form which you can download below. Please send the completed and signed form to the Department of Public Service.
