PICA First Time Child Passport

The parent/ legal Guardian is required to obtain and complete an application form for the Minor. The Jamaican passport is valid for ten years for an adult and five years for children 17 years and under.

Along with the application form the following documents will also be required :
** The Minor’s Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate
** Two (2) identical Photographs ( NB// : one photograph must be certified by the same certifying official who certified the application)
** A valid Government issued Photo Identification or passport for the parent/legal guardian (Example : Drivers Licence, Voter Identification).

** Proof of name change if name has been changed( Deed poll or Court Order)
** The parent/legal guardian submitting the application is required to complete sections C and E of the application form.

Renewing a Child Passport

The parent/ legal Guardian is required to obtain and complete an application form for the Minor.

Along with the application form the following documents will also be required :
** The Minor’s Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate
** Two (2) identical Photographs ( NB// : one photograph must be certified by the same certifying official who certified the application)
** A valid Government issued Photo Identification or passport for the parent/legal guardian (Example : Drivers Licence, Voter Identification).
** Proof of name change if name has been changed( Deed poll or Court Order)
** Current passport
** The parent/legal guardian submitting the application is required to complete sections C and E of the application form.

1. Please note that all Minors 3 years and older must appear in person when the application is being submitted and must be accompanied by the parent/legal guardian who completed the application form. Where a parent/legal guardian has a difficulty visiting one of PICA’s location/Overseas Mission or The Customer Service Unit should be contacted for further advice.

2.Consent for a child ( Section C of passport application form)

3.The mother, father or legal guardian must complete the application for a child less than 18 years of age. Persons under 18 years of age, who are unmarried, require the written consent of the mother, father (providing his name is stated on the birth certificate) or legal guardian, except where such person is a member of the Security Forces. In the event that neither parent nor legal guardian is available, contact should be made with the Customer Service Unit at PICA or the nearest consular representatives in the overseas missions.

Replacing a lost/stolen or damaged minor (under 18yrs) passport :
Your will be required to obtain and present a police report for the replacement of your lost/stolen or damaged passport.

You will be required to submit the following documents :
** Completed application form
** The official police report

** Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate for the minor
** Two (2) identical Photographs ( NB// : one photograph must be certified by the same certifying official who certified your application)
** Proof of name change if name has been changed (Marriage Certificate, Deed poll, Divorce Decree, Court Order)

** A valid Government issued Photo Identification for the parent/legal guardian (Example : Drivers Licence, Voter Identification).
** The parent/legal guardian submitting the application is required to complete section C of the application form.

Note :
If the parent/Legal guardian is out of Jamaica then a written letter of consent giving the guardian permission to apply for and collect passport (notorized by an authorised person from the country they are in) is to be done and sent along with a copy of their passport or acceptable ID to the adult who will be doing the application through our office.

** Overseas applications (submitted through the Jamaican Missions) – twenty (20) working days
** Express Services Available from our Kingston Offices Next Day and Three working days
