Ontario Accessible Parking Permit

How to get an accessible parking permit. Vehicles used by people with disabilities need to display the permit to park in an accessible parking space.

You can visit a Service Ontario to get a temporary accessible parking permit valid for 90 day.

There are 2 parts to the application form :
** section A – to be completed by you
** section B – to be completed by your healthcare practitioner

Follow the steps below to fill an online request form :
1.complete and print section A of the online request form
2. print section B of the form and ask your healthcare practitioner to complete it. A licensed physician, chiropractor, registered nurse practitioner (extended class), physiotherapist, occupational therapist, chiropodist or podiatrist must certify your condition as permanent, subject to change or temporary on the application.

bring the completed sections A and B to a ServiceOntario Centre or mail it to :

You can also get an application form at any ServiceOntario Centre.

Delivery : 3 weeks by mail

Note :
To use until your regular (blue) or short-term (red) accessible parking permit arrives by mail.

What you need to know :
There is no age limit to apply for an accessible parking permit. You can use an accessible parking permit on any vehicle you will be travelling in, even if you do not own a car or have a driver’s licence.

Your healthcare practitioner will indicate which of the 5 types of accessible parking permits you are eligible for :
If you have a permanent disability :
you can get a permanent permit (blue). It is valid for 5 years and can be renewed without recertification from a health practitioner.

If you have a disability that is considered subject to change :
you can get a subject-to-change permit (orange). It is valid for 5 years. To renew this permit you are required to be recertified by a health practitioner.

If you have a short term disability (lasting between 2 months to 1 year) :
you can get a short term permit (red). This permit is not renewable.

If you are travelling outside Ontario :
you can get a traveller’s permit (purple) to park your car in an Ontario airport for up to 1 year while you use your regular permit during your trip.

If you are a company or non-profit organization :
you can get a company permit (green) for every vehicle of your company or non-profit organization that is used mainly to transport people with disabilities. It is valid for 5 years and can be renewed.

Renew your Parking Permit

** You will need to renew a permanent and subject-to-change accessible parking permit every 5 years. You will receive a renewal notice 3 months before your permit is set to expire.
** If your disability is permanent : you don’t need to include more information from your healthcare practitioner.

Follow the steps below for renewal and indicate any changes to your personal information in the application :

If your disability is subject to change : your healthcare practitioner will need to provide more information in your renewal application. Follow the steps below for renewal and make any changes to your personal information in the application:
** complete and print section A of the online form
** print section B and ask your health care practitioner to complete it

Replace a damaged Parking Permit

There are 2 steps to replace a damaged accessible parking permit.

You can get an application form at any ServiceOntario Centre or you follow the steps below :
** complete and print section A of the online form

bring the damaged accessible parking permit with the completed form to a ServiceOntario centre or mail it to :
ServiceOntario
P.O. Box 9800

If you can’t return the damaged permit, you need to apply for a lost accessible parking permit.

Replace a lost or stolen accessible parking permit :
There are 4 steps to replace your lost or stolen accessible parking permit.

You can get an application form at any ServiceOntario Centre or you follow the steps below :
** complete and print section A of the online form
** print a replacement declaration

complete the replacement declaration by following 1 of the options :
** have your local police department complete the declaration
or
** call your local police department to report your lost or stolen parking permit. You will need to get an occurrence number and badge number.
