Conditions and Requirements

** The Emirati’s wife should be sponsored by him and registered in the family book.
** A medical card will not be issued if husband passed away or wife is divorced even if she is sponsored by her Emirati children.
** The customer should have a valid residency visa.
** Issuing the medical card is linked to the place of residence.
** “To Whom It May Concern” Letter issued by the company stating that the customer resides in Dubai or the company housing complex.
** Medical cards of Emiratis are valid for 4 years.
** Medical cards of Emiratis and nationals of GCC States are valid for 4 years. Expatriates medical cards are valid only for one year.
** Proof of residing in the Emirate, such as tenancy contract, electricity bill or house ownership certificate.
** A person is issued one medical card only.
** When applying for the medical card via e-system, the application form and receipt should be printed out and be submitted along with the personal photos to the Medical Registration Section.

Required Documents

1. A copy of valid passport.
2. A copy of valid Emirates ID card.
3. Personal photo with white background.
4. A proof of residence in the Emirate.
5. Filling out the Smart Medical Card application.
6. A copy of the Emirati’s family book.
7. A certified copy of the marriage certificate if the spouse is not an Emirati.
8. A valid residency visa or entry residence visa for expatriates arriving for the first time.
9. A copy of the diplomatic passport, a copy of the card issued by the Ministry of Foreign Affairs in yellow and a letter issued by the Consulate for the category of diplomat confirming that the person is still working in the Consulate.

Procedures and Steps

1. Printing out the Smart Medical Card Application through the e-system or printing offices and paying the fees.
2. Referring to Customer Service Centers to submit the application.
3. The concerned officer will validate the original documents and review the information stated on the application form.
4. Issuing the new medical card.
5. The customer receives the medical card

Service Fees

** Locals (0 to 10 Years) 25 AED
** Locals (11 to 18 Years) 50 AED
** Locals Above 18 Years 100 AED
** Non Locals 500 AED

There is an additional fee depending on the type of card used to pay
