Conditions & Requirements

1. The presence of a relative of the deceased to extract the certificate.
2. A copy of the death certificate, if any

3. A proof card of the relative of the deceased.
4. The passport or a copy hereof.

5. In the case of companies, the presence of the company representative with a letter from the company.

1. A copy of the death certificate, if any
2. A copy of passport (in the case of expatriates, it is required to submit valid residence).

3. A copy of the registration record, with the original version for nationals
4. A letter from the cemetery with the name and date of the deceased person.
5. The presence of a relative of the deceased for nationals and non-nationals or power of attorney.

6. Death notice or forensic report.
7. A letter from the police station of the loss of the death certificate
8. An authenticated copy of the notice of the hospital that issued it.

2. Inserting the application in the electronic system and paying the fees.

3. Verifying the documents and original copies, and issuing the death certificate

FAQs

Do I need to conduct a premarital medical test If I’m non-Muslim?
Yes, it is still necessary to undergo a premarital medical test & give the report to the responsible person so the procedure of marriage can be completed.​

What are the necessary procedures in case the patient’s results show the presence of infectious diseases?
** We recommend the importance of preventive measures and vaccinations and make sure the second party knows about the disease.

** The patient also should complete the consultation before taking any step.
** If the parties wish to complete the marriage despite the risk, a report may be issued after completion of the necessary advice and signatures.

** Transferring the patiant to specialized clinics affiliated to the MOHAP is important to complete the necessary analysis and treatment and long-term follow-up.
** Inform the preventive medical parties in the area to carry out epidemiological surveillance if necessary, while also providing specialized consultations for cases that warrant this.

** Provide the necessary preparations for illnesses like German measles and hepatitis B, especially for people who are at risk of infection and who are constantly mixed with the patients.

** Ensure complete awareness for healthy and healthy pregnancy.

Are there any extra fees if I need to receive the reports more quickly?
​The medical report of the premarital test is often issued within three working days, there is no urgent option to request the examinations. Therefore, you are advised to set aside a sufficient amount of time for undergoing the medical test.​

I work in the army and it’s difficult to obtain a leave permission form,can my brother or a relative sign the medical report & pre-marital counseling?
It’s preferred that you receive the results yourself, otherwise you can delegate someone to receive the reports only if the results are clean.
