Duplicate Death Certificate & Update Death Register :

Obtain a Certified Copy of the Certificate of Death :
1. Refer the application to the Divisional Secretariat of the area where the death is occurred.
2. Application can be obtained from any Divisional Secretariat.

3. Fees chargeable
** Where the date of registration or the no. of the entry is given the fee for one copy of the certificate is Rs. 100.00
** Where the date of registration or the no. of the entry is not given and a search of registers not exceeding two years is involved fee for one copy of the certificate is Rs. 200.00

4. Required fee should pay to the Divisional Secretariat.
5. Duly filled application should submit to the Divisional Secretariat.
6. If you like to obtain the certificate of death by post, submit a stamped envelop with the application.
7. If the death is registered a certificate of death will be send and if the death is not registered a letter to the effect will be send.

Alteration of Information in a Death Register :
1. Produce the declaration to the Additional District Registrar the Divisional Secretariat of the area where death occurred.
2. The declaration should be produced by;
** The informant
** Trustworthy person knowing the truth relating to the incident

3. All information other than the date of death may be altered.
4. Fee chargeable is Rs. 50.00
5. Written evidence should be produced in proof of accuracy of the request made in the declaration.

Obtain a copy of death certificate from divisional secretariat :
The ordering method :
1. Getting a formal application form
** The applicant must fill in the application form by a valid application form from the relevant

Registrar General or any divisional secretariat :
** If the death occurred within 3 months of the occurrence of the death, the following letters should be submitted.
** If the GCE A / L certificate is at home or at death

** Medical Certificate of Medical: If death occurred in a hospital
** The following data should be provided when registering after the expiration of more than 3 months of occurrence of the change.
** Letter of request
** Warning ad

** The witnesses who witnessed two names
** Parks or pastors participating in religious rooms (on the land)
** If he or she is supported by a funeral fund, a society or other members of the office,
** If the fire hit suddenly, it is searched by the Accidental Section
** Application Form No – Hometown / Year / Month
** Normal Registration Form – 63A
** If you are making a late registration – Form 15

2. Handing over the Handged Application Applicant can visit the Registrar’s office and hand over application form or a stamped envelope to the Registrar.
