Surrey County Blue Badge application

** A Blue Badge is normally valid for three years and costs £10.
** You can expect your badge to arrive within four to six weeks once approved.

** You can apply for yourself or on behalf of another person or organisation.
** The badge holder can use it in any vehicle, whether they are the driver or a passenger.
** It can be used throughout Europe.
** Blue Badges are clearly printed with an expiry date, if it has expired please cut it up and dispose of it, or if it is no longer required, please return it to us immediately.

Continue your saved application :
** You can start and save your Blue Badge application online.
** You will be given a tracking number which will allow you to continue and submit your application within two weeks.
** Any online applications that are not submitted within two weeks are automatically removed from the system.

Why apply online :
It is quicker and easier to apply for a Blue Badge online because you can :
** Immediately check that you are eligible.
** Upload your photo, documents and pay for your badge (payment is only taken if your application is approved).
** We will contact you if you need a walking assessment, or if your application was not successful.
** If you are unable to complete the transaction online you can apply for a Blue Badge by post.
** If you need to contact us, please provide us with your name and address and if appropriate your Blue Badge reference number.

Apply for or renew a Blue Badge by post :
We recommend applying for a blue badge online as this allows you to :
** Immediately check that you are eligible.
** Submit your details directly to the council responsible for issuing your badge if you qualify.
** Upload your photo, documents and pay for your badge (payment is only taken when your application is successful).
** A friend, relative or representative can apply online on your behalf with your permission.
** You can only apply to us by post if you pay your council tax to Surrey County Council.
** If you pay your council tax to another authority you can still apply online and your application will be sent to the correct local authority.
** Alternatively, you can contact your local authority for their postal application forms.

Step 1: Get the application form and notes
You can download the appropriate application form and notes below :
** Application form for individuals
** Application form for organisations
** The forms and notes are also available from Surrey County Council libraries and from help shops.

Step 2: Read the notes and complete the form
** Read the notes that come with the form to check that you are eligible for a Blue Badge.
** Please remember to sign the form.

Step 3: Gather your supporting documents
** You will need to send us some documents with your completed form.

To prevent any delays, make sure that you have all of the following :
Proof of identity (for new badges only): An endorsed photocopy of an identity document – do not send us original documents.

Proof of address :
** A photocopy of a recently dated document with your address – do not send us original documents.
** Any other documents that we have requested in the form. Photocopies of any documents to prove your eligibility – do not send us original documents.
** Payment. Blue badges cost £10. Cheques should be made payable to Surrey County Council.
** Photograph. A passport sized colour photograph, taken within the last six months, that shows your face clearly.
** Check the notes that accompany the application form for guidance about which documents are acceptable for proof of identity and proof of address.

Step 4: Send the form and documents to us
** Send your form, documents and payment to- Blue Badge Team, County Hall, Penrhyn Road, Kingston-upon-Thames KT1 2DN
** Include a stamped addressed envelope for an acknowledgement.
** If you need further assistance you can call our contact centre.
** It will normally take between four to six weeks to process your application.
** We will contact you if we need any more information or if you need a walking assessment.
** We will also contact you if your application was not successful.
