Vital Stats Apply for Birth Death & Marriage Certificate

Who can apply for certificates :
Birth :
** You, if the application is for your own certificate.
** Parents named on the record of a child.
** Guardian (must submit guardianship papers).

** A representative with written authorization of a) b) or c) above.
** Written authorization of person entitled to certificate must accompany the application.

Marriage :
** Either party to the marriage.
** The children or parents if both parties are deceased.
** A representative with written authorization of a) or b) above.
** Written authorization of person entitled to certificate must accompany the application.

Death:
** Any person with a valid reason.
** Only next-of-kin may obtain a certified photocopy of a death registration.

Fees & Services

For each certificate issued by the director, including the search – $30.00
For each certified copy of a registration or extract from a registration issued by the director, including the search – $30.00
For each commemorative certificate issued by the director – $30.00

For a search and results of the search :
of restricted records – $30.00
of unrestricted records – $12.00
if the client searches the records and identifies the registration by its electronic registration number – $12.00
if the director searches the records – $30.00
For a registration of birth, marriage, death or stillbirth not registered within one year of the date of the event – $30.00

For re-registration of the birth of a child where there is a decree of adoption or an adoption order – $30.00
For a correction of an error in registration – $30.00
For a legal change of name including Manitoba Gazette fee – $120.07
for a legal change of name for each additional family member included in the same application – $50.00

Rush service to send a certificate, certified copy or search and report by the director to :
Canadian destinations – $65.00
USA destinations – $75.00
Other destinations – $105.00
Administration fee retained when customers do not respond to requests for additional information – $30.00

** As of December 15, 2015, the Vital Statistics Agency will only offer 3-business-day rush service to process an application for a birth, marriage, death, or stillbirth document (certificate, certified copy, or search and report).

** This service is only available for fully registered ready-to-issue vital events.
** Delivery time is additional.
** Rush service fees remain unchanged.
** If additional steps and time are required to make the application ready-to-issue, the application resets to Day 1 on the day the application is made ready-to-issue.

How to Apply

** All the appropriate sections of the application form must be completed in full.
** Incomplete applications or those with insufficient payment will be returned.
** Documents can be issued only if the birth, marriage or death occurred in Manitoba.

** Certificates contain information extracted from the original registration filed at our office.
** Birth certificates are issued in the applicant’s birth name, unless a legal change of name has occurred.
** If you have changed your name upon marriage, please provide your name before your first marriage.

** Counter service is available at 254 Portage Avenue, Winnipeg.
** The building is located at the SW corner of Portage Avenue and Garry Street.
** The Agency is open Monday through Friday, from 8:30 A.M. to 4:30 P.M.
** Available payment methods are cash, cheque, money order, debit, Visa or MasterCard.
** Please note personal cheques, unless certified, will NOT be accepted for RUSH service.
