Apply for an e+ card :

How to apply :
** It is possible to do the first part of your enrolment online at the e+ single enrolment website.
** You can add your personal information and your choice of services.

Apply for an e+ card online :
** To complete the process, you will need to visit one of the Bracknell Forest Council e+ sites where your photograph will be taken by a member of staff and your personal details will be checked against your verifying documents.

** Online applications can be completed at any of the sites listed below except Downshire Golf Complex.
** You can also apply directly without the online application at the reception desk at Time Square.
** Alternatively, call in to your local Bracknell Forest library or visit Bracknell Leisure Centre, Edgbarrow Sports Centre, Sandhurst Sports Centre or Downshire Golf Complex.

** Please note that if you are disabled and would like to use your e+ card as a concessionary bus pass, you will need to bring your documentary proof of disability into our Time Square council office to complete your application.
** A member of staff will take you through a short application process and then take your photograph.
** This photo will appear on the front of your e+ card to ensure only you can receive the great benefits the e+ card brings.

Documents to bring with you :
** If you are not an existing Bracknell Forest library or leisure member you will need to take two forms of identification with you – one showing your address and the other with your signature on it.
** If you are over 60 and applying for a concessionary bus pass, you will also need to bring proof of your age.
** If you are applying for a bus pass and are disabled, you will need to show us proof of a permanent disability.
** If you are a young person applying for an e+ PASS proof of age card you will need to provide photographic proof of age.

What happens next :
** When the enrolment process has been completed, your card will be sent to you by post ready for use.
** Your first e+ card will be issued free of charge.

Updating your details :
** If you already have an e+ card you can log into your e+ account to update details such as your telephone number and email address.
** For changes to your name, address or photo please visit us in person with proof of the change, where necessary.

Lost your e+ card :
** If you lose your e+ card, we can replace it for a £5 fee.

You may be able to order a replacement by telephoning customer services on 0845 601 1632 if :
** you have a standard e+ card
** your address and name details have not changed
** your photograph does not need updating
** you are able to pay the £5 fee with your debit or credit card over the phone

However, we will need you to call in to an e+ site in person if :
** the lost e+ card you need to replace is a concessionary bus pass or a PASS proof of age card
** we need to see documentary proofs
** we need to take a new photograph of you
** If so, please bring any necessary proof documents with you and pay the £5 fee at the site you visit.
