Replacing a Lost or Stolen Passport :

** If your passport has been lost or stolen, you must obtain a Statement of Lost Passport from the Exit & Entry Bureau (EEB), which is under the Public Security Bureau (PSB), before you can apply for a replacement passport.

** Once you have obtained the replacement passport you will need to update your registration with the Chinese government, either by contacting your local police station or your hotel (hotels/hostels will register their guests with the police directly).

** You must then bring your Statement of Lost Passport, updated registration information, and your new passport to the EEB again to obtain an exit visa.

** Although the EEB Statement and updated registration information by themselves are enough to obtain a replacement Chinese visa, in many cases the EEB will expedite a request for a Statement if you submit a report from the local police office where the loss occurred.

Please Note :
** Our office can assist only in issuing a replacement passport.
** Following the issuance of a new passport, the Chinese government requires that a new visa be obtained for the replacement passport.
** The same EEB offices (see addresses above) can issue new visas for replacement passports.
** Failure to obtain a replacement visa can result in a fine or detention by law enforcement authorities.

** All replacements for lost or stolen passports must be applied for in person – no appointment is required.
** If you have time to wait for the processing time (usually 10 working days), you may apply for a full-validity replacement passport.
** if you are in urgent need to travel, you may apply for an emergency replacement passport with a limited validity.

Please note :
** As emergency passports are issued for short validities, you will need to replace your emergency passport with a full-validity passport before you will be eligible for many kinds of replacement visas.

** In most cases, a Chinese resident visa (such as a one-year work visa) will only be issued to a full-validity passport, and the Chinese government requires a passport to have at least six months of remaining validity for the bearer to be eligible for entry to China.

Please bring the following to your appointment :
** Statement of Lost Passport from Exit & Entry Bureau (EEB)
** Passport Application form DS-11
** Statement Regarding a Lost or Stolen Passport form DS-64
** Proof of identity (e.g. U.S. driver license, U.S. state ID, student ID, and/or Military ID)
** If available, a photocopy of your lost or stolen passport, or other proof of U.S. citizenship

** (e.g. an official birth certificate or naturalization/citizenship certificate, a Consular Report of Birth Abroad, and/or an expired passport)
** One Passport Photo (must be 2″ X 2″ (5 cm X 5 cm) with a white background, and taken within the last six months)

Note :
** All applicants under the age of 16 must meet the requirements listed in the Law on Passport Applications for Minors.
** In most instances this means that both parents must sign the child’s passport application, even if one parent is not an American.
** All parents or guardians must present proof of the child’s birth or adoption to verify their relationship to the child.

Parents can consent in one of two ways :
** Both parents, even if one is not an American, can come to the Consulate with their passports and sign in person.
** If one parent is not present in China, s/he may give his/her consent through a written affidavit, the DS-3053 form.
** The form must be notarized by a notary public in the U.S or at a U.S. embassy or consulate within the previous three months.
** Please note that the parent who signs in person will need to still have a copy of the non-present parent’s identification with him/her.
