How To Do Nova Scotia Birth Certificate Registration?

** Every child born in Nova Scotia must be registered with Nova Scotia Vital Statistics within 30 days of the birth.
** The registration establishes a permanent legal record of the birth event.

Steps To Register:

** Nova Scotia Vital Statistics is offering a new bundled birth service in several hospitals across the province.
** You can electronically submit your baby’s birth registration information using kiosks located in each regional hospital and the IWK.

When you complete the electronic birth registration form you can apply for the following services at the same time :
** The baby’s Social Insurance Number (SIN) with Service Canada;
** Canada Child Benefits (CCB) with the Canada Revenue Agency (CRA);
** Activation of the baby’s Nova Scotia Health Card; and,
** The baby’s Nova Scotia Birth Certificate (fee).

** If you do not give birth in one of the regional hospitals, or at the IWK, a paper registration form is also available.
** However, the paper form only integrates the applications for the Social Insurance Number and Canada Child Benefits into the Nova Scotia Birth Registration process.

** When the birth registration form is completed, it is submitted to the Local Division Registrar at the hospital, who reviews, approves and forwards it to the Vital Statistics Section, Service Nova Scotia.

** Additional forms may be required to legally acknowledge a father or other parent on the birth record.

These forms must be witnessed by either :
** Division Registrar of Births; or
** A Commissioner for Taking Oaths; or
** Barrister of the Supreme Court; or
** Notary Public

** A Division Registrar is available in each hospital to assist you with this process.
** If more than a year goes by before a birth registration document is filed with Nova Scotia Vital Statistics,

Then :
** A Delayed Registration of Birth will have to be filed; and
** Evidence of the birth will have to be produced; and
** There will be a fee for the delayed filing, please visit Vital Statistics Fees for fee amount.

Birth Certificate :
** There are restrictions on who can apply for a birth certificate.
** Vital Statistics will issue a birth certificate to the person named on the certificate, a parent or legal guardian or the administrator of the person’s estate.

Certificate Formats

** To ensure the correct certificate format is obtained, please contact the organization that requires the birth certificate to determine if a short or long form is needed.
** Some common uses include school registration, passport, social insurance numbers and driver’s licences.
** Please note that for married women, birth certificates are issued in the maiden name.

Short Form :
The short form birth certificate contains the following information :
** surname and given name(s)
** date of birth
** place of birth
** sex
** registration date
** registration number
** date issued

Long Form :
** The long form birth certificate contains all the information of the short form birth certificate plus the parent’s names and their birthplaces.
The long form birth certificate contains the following information :
** surname and given name(s)
** date of birth
** place of birth
** sex of child
** name of parents
** birthplace of parents
** registration date
** registration number
** date issued

Eligibility For Birth Certificate

Who qualifies for a birth certificate? (Effective February 15, 2002)
** You, if the record pertains to your birth
** Parents of a child
** A lawyer who specifically indicates they are working on behalf of “A” or “B” above, or a person on the written authorization of “A” or “B” above
** The executor/executrix or trustee for “A” or “B” above
** Guardian (copy of guardianship papers must be attached to the application)
