Instructions For Candidates :
1. The attested Photo copy of Identity Card must be attached with the application.
2. The application must be completed in all respect.
The application will be rejected and fee will be forfeited if it is not submitted within three months.
3. No other person can Submit the application form and not receive the certificate in lieu of other person the certificate will be dispatched by registered post.
4. The fee for consolidated certificate is as :
Ordinary Rs.2070/-
Urgent Rs.3070/-
