Register a Birth

** We register all babies born in Queensland—complete the birth registration application form within 60 days of your baby’s birth.

** We do not send a birth certificate when a birth is registered—you will need to buy one.

You may need your child’s birth certificate when you are applying for :
** entry to pre-school, primary or secondary schools
** an Australian passport.
** You can choose to register your baby’s birth and pay for a certificate online—or you can choose to complete the birth registration application and the birth certificate/extract application form and submit them to us by post or in person.

** When applying online, only complete the online application form once.
** We will only process the first registration application that you submit.
** Processing your baby’s registration and birth certificate application forms can take up to 10 business days.

** If you apply for a birth certificate when you register your baby’s birth (you can do this online), we will post the certificate to you.
** If your baby was delivered before 20 weeks or weighing less than 400g—we are not able to register the birth but you do have the right to commemorate your family’s loss with an early pregnancy loss recognition certificte.
** In the case of same sex female parents—only the birth mother can be named as the mother, the female partner must be named as the parent.
** If your child was born prior to 1 June 2010 and you would like to add a same sex female parent contact us.**

How to apply :
To register a birth and apply for a birth certificate :
Decide how you want to apply :
** Register a birth with an electronic signature—you can also pay for a birth certificate (if required).
** You will need your wallet or purse with you to verify your identity, and to pay by credit card if buying a birth certificate.
** Register a birth and print to sign—you can also pay for a birth certificate (if required).
** You will need to get another adult to witness your signatures, then deliver to us by post or in person.
** Download the birth registration application form—to register your baby’s birth you will need to print, complete and sign your application,

** complete the birth certificate application form and pay for it (if required) and deliver to us by post or in person. You will need to get another adult to witness your signatures.
** Urgent certificate delivery services are not available when registering your baby.
** There is no fee to register your baby if it is registered within 60 days.
** A late registration fee of $4.80 applies for a birth registration application form received after 60 days from your baby’s birth.
** If you order a birth certificate with the birth registration, a fee is payable.

By post :
** If you choose not to complete the register a birth and apply for a certificate online, post your birth registration and certificate
application forms to :

City East Qld 4002.

In person :
** If you choose not to complete the register a birth and apply for a certificate online, visit the Brisbane registry counter at 110 George Street, Brisbane with your completed application forms to register your baby and apply for a birth certificate.

** If you do not live in Brisbane, you can register your baby and apply for a birth certificate at your nearest Queensland Magistrates Court or Queensland Government Agent Program (QGAP) office. Your certificate will be posted to you.

** The total cost is calculated when you fill in the application form online.

You can pay online :

** by EFTPOS or credit card

** Make cheques and money orders payable to ‘Registry of Births, Deaths and Marriages’.
