Obtaining Copies of Fetal Death & Still Birth Records :

** CDPH Vital Records only accepts orders submitted by mail.
** Please follow the 5 Steps listed below to place your order.

Step 1 :
** Download pamphlet for obtaining certified copies of fetal death and still birth records

Step 2 :
** Download and complete the application form
** Application for Certified Copy of Fetal Death Record (PDF)Opens a new browser window.
** Application for Certified Copy of Still Birth Record (PDF)Opens a new browser window.

Step 3 :
** Determine certified copy fee
** A certified copy fee must accompany all requests for copies of vital records.
** Requests received without the appropriate fee will be returned to the sender.
** Make your check or money order payable to CDPH Vital Records.
** Checks must be drawn on a United States bank.
** Money orders must be drawn on a United States bank or issued by the United States Postal Service.
** Please Do Not Send Cash By Mail.
** CDPH cannot be held responsible for fees paid that are lost, misdirected, or undelivered.
** If no record is found, we will issue a Certificate of No Public Record and retain the fee for the search according to State law.

Step 4 : Vital record fees
Vital Record Fees :
** Please send a check or money order made payable to CDPH Vital Records.
** Please Do Not Send Cash By Mail.
** CDPH cannot be held responsible for fees paid that are lost, misdirected, or undelivered.

** Death Certificate – $21
** Fetal Death Certificate – $18
** Public Marriage Certificate – $15
** Dissolution of Marriage Certificate – $14
** Still Birth Certificate – $24

Amendments :
** Acknowledgement of Paternity – $23
** Adjudication of Facts of Parentage – $23
** Adoptions – $20
** Affidavit to Amend a Birth Certificate – $23
** Affidavit to Amend a Death Certificate – $23
** Affidavit to Amend a Marriage Certificate – $23
** Correcting Genders Errors – $23
** Court Order Name Change – $23
** Gender Reassignment – $23

Delayed Registrations :
** Delayed Registration of Birth – $23
** Court Order Delayed Certificate of Marriage – $23
** Court Order Delayed Registration of Birth – $23
** Court Order Delayed Registration of Death – $23
** Court Order Delayed Registration of Fetal Death – $23

Step 5 :
** Mail your request to Vital Records
** Include completed application form, and check or money order for the certified copy fee.*
** Please Do Not Send Cash By Mail.

Note :
** Concerning Confidential Information on Fetal Death Record

** The bottom portion of the Fetal Death certificate (entitled “Confidential Information for Public Health Use”) contains confidential personal information (race, occupation, and medical data – including pregnancy history).

** Health and Safety Code Section 102430 allows this confidential information to be included on an authorized copy only when specifically requested by the mother listed on the certificate.
** Only the mother or father may request a certified copy of a Still Birth record.
