1. Supporting documents necessary for adult passport issue:
a. An application form provided by the Passport Office which also acts as a Statement of Truth in which the applicant states whether he/she:
** has been convicted of forgery, counterfeiting of certificates, embezzlement of documents, false testification or false declaration, given that these offences are in respect to the issuance, use, loss or theft of a passport.
** has been prosecuted for abduction, human trade, trafficking, kidnapping, child abduction, slave trading or smuggling of illegal immigrants.
** has been prosecuted or set to go to trial for the aforementioned offences.
** has been declared a draft-evader or deserter or overseas deserter.
** has a pending prohibition to exist the country,
** resides permanently at the stated address and if he/she has been issued a passport, in accordance with the provisions of the Law 3103/2003, which may have been lost, stolen or if a renewal or replacement passport is being requested.

b. A current (month-old) colour photograph, 4×6 mm in size and meeting technical specification criteria:
In cases of persons with vision problems, when the photo specifications do not fulfilled exactly (for example not focus, closed eyes, facial deformity and others), submitted as supporting document, or simple certificate of registration in blind’s records, which certifies the above condition, or medical certificate.

c. Receipt of deposited fees:
1.Stamp deposit:
** For a 5 year expiration date passport, a 22 euro nominal value receipt +20% (Organization of Rural Insurance)= total value 26,40 euro (applicants over 14yrs of age).
** For a 13 month expiration date passport, a 9 euro nominal value receipt+20% (ORI) = total value 10,80 euro.(For passports issued in accordance with Article 1 §3 P.D 25/2004 and Article 4 §3 P.D 25/2004 as replaced with Article 3 P.D 135/2007)
** For a 8 month expiration date passport, a 4,5 euro nominal value receipt +20% (ORI) =total value 5,40 euro.(For cases where fingerprinting is currently unavailable)
** For a 3 month expiration date passport, a 4,5 euro nominal value receipt + 20% (ORI) = total value 5,40 euro.(For passports issued in accordance with article 1 §4 case b P.D 25/2004, as replaced with Article 1 § 4 P.D 30/2010)

2.Passport booklet Fee 53,00 euro

3.Postal fees 5 euro

4.Photocopy of both sides of identity card ( in the case of an overseas resident who does not have a Greek identity card, the applicant must submit their birth certificate or a certificate of family status (validity last 6 months).

2. Supporting documents in case of passport renewal or replacement:
The aforementioned (case 1) supporting documents are submitted as well as the old passport.

3. Supporting documents in case of passport theft or loss:
In issuing a new passport the same supporting documentation stated in case 1 must be submitted as well as recent certification from the Hellenic Authorities (Police or Consulate) declaring the theft or loss and on which the results of the conducted investigation are stated.

4. Supporting documents required for a minor’s passport:
An application form is required and can be obtained from the Passport Office Information desk. The application form acts as a Statement of truth. For an applicant under the age of thirteen (13) the legal guardian must certify the minor’s stated address and if a passport has been previously issued in their name, according to the provisions of the Law 3103/2003.

An applicant over thirteen (13) years of age must state if he/she:
** has been convicted of forgery, counterfeiting of certificates, embezzlement of documents, false testification or false declaration, given that these offences are in respect to the issuance, use, loss or theft of a passport.
** has been prosecuted or set to go to trial for the aforementioned offences.
** has a pending prohibition to exist the country.
** resides permanently at the stated address and if a passport has been previously issued in their name, according to the provisions of the Law 3103/2003.

Receipts of Fee deposit:
1.Stamp deposit:

4.Photocopy of both sides of applicant’s identity card
5.Birth certificate or Family status certificate (issued within the last 6 months) for applicants under twelve (12) years of age who do not have an identity card. A recent colour photo must be affixed on the certificate and must be validated by the local authorities where the applicant is enrolled.
6.A recent colour photo (taken within the last month), 4x6cm fulfilling required technical specifications.

In cases of persons with vision problems, when the photo specifications do not fulfilled exactly (for example not focus, closed eyes, facial deformity and others), submitted as supporting document, or simple certificate of registration in blind’s records, which certifies the above condition, or medical certificate.

The aforementioned supporting documents are submitted and signed by the parents and in the minor’s presence who, if over twelve (12) years of age, must also sign. If only one parent is present, the written consent of the other must be submitted with their validated signature.

If, due to reasons beyond their control, parents or legal guardians cannot be present a relative must be present with the written authorization of one parent and written consent of the other parent. Signatures have to have been validated by local authorities. If parents are unable to authorize a relative, a guardian is appointed and the supporting documents are submitted as specified in Articles 1601 and 1602 of the Civil Code.

General Information:
Supporting documents submitted must be recent and have been issued within the last six (6) months (Article 2, par. 2, Common Ministerial Decision 3021/22/10).

The stamp fees and receipts of deposited fees can be obtained at the Public Financial Service Office and authorized tax offices.

If the municipal registrar number is not specified on the applicant’s identity card this can be declared orally and completed on the passport application form.

Upon submitting all necessary documentation the applicant must have their identity card with them, for identity confirmation purposes.
