DMV Get & Replace a Learner Permit

Get a learner permit :
** To get a New York State learner permit you must be 16 or older and apply at a DMV office.
** Before you visit the DMV, study the NYS Driver’s Manual and take the practice tests so you can pass the written test.

** You will also need to take an eye test, unless you bring an Eye Test Report (MV-619) completed by an eye-care professional.

Bring to the DMV :
** a completed Application for Permit, Driver License or Non-Driver ID Card (MV-44)
** if you are under 18, your parent/guardian must sign the ‘consent’ section (they don’t need to sign it at the DMV)
** if you are 17 and have a Driver Education Certificate (MV-285), you don’t need consent
** your Social Security Card (photocopies are not acceptable) or proof of ineligibility 1
** acceptable proof of your identity and date of birth

** if you are under 21 and use the Statement of Identity by Parent/Guardian (MV-45) as proof of identity (you cannot use as proof date of birth), your parent or guardian must sign it at the DMV, and they must present 6 points of proof of identity

** payment for the fees.

About the written test :
** You must take a written test to receive your learner permit.
** To pass the test, you must get least 14 of the 20 multiple-choice questions correct.
** This includes the correct answers to at least two of the four questions about road signs.
** If you are applying for a motorcycle permit, you must get at least three out of the five motorcycle questions correct.
** If you do not pass the written test, you can take it again for no additional fee.
** There is no limit to how many times you can take the written test.
** If you pass the written test, we will issue you a learner permit that is valid for approximately 5 years.
** The exact expiration date depends on your date of birth and the date that you applied for the learner permit.

After you visit :
** If you are not eligible for a Social Security Number, you must give us a letter from the Social Security Administration indicating you are not eligible.
You can pay by :
** cash
** credit
** debit card (it must not require a personal identification number (PIN))
** check or money order made out to “Commissioner of Motor Vehicles”

Replace license or permit :
** You can replace your license or permit if it was lost, stolen or destroyed, or after you update your address.
** You can’t replace your document if it is expired.
** If you are turning 21 soon, order within 40 days of your birthday.

Can I order online?
Yes, unless
** you are 25 or older and have ordered replacement online within the past 30 days
** you are under age 25 and ordered a replacement before
** you have a conditional or restricted driver license
** your document says “Valid Without Photo”

How do I order by mail?
** Read How to Apply for a New York Learner Permit, Driver License, Non-Driver ID Card (MV-44.1).
Then :
** complete an Application for Permit, Driver License or Non-Driver ID Card (MV-44)
** if you do not know your DMV ID number, enter your date of birth and name as it appeared on your old document
** provide photocopies of acceptable proof of identity
** include payment for the fee (check or money order payable to “Commissioner of Motor Vehicles”)

How do I order at an office?
** complete an Application for Permit, Driver License or Non-Driver ID Card (MV-44)
** if you do not know your DMV ID number, enter your date of birth and name as it appeared on your old document
** bring acceptable proof of identity (you must provide the original documents)
** be prepared to pay the fee
** A local DMV office cannot issue a document with your photo.
** You will be given a temporary, non-photo document to use until you receive your new one in the mail.

What is the fee?
** The fee is $17.50.
** If you want the fee waived for a license or permit lost due to a crime, you must apply at a DMV office and show form MV-78B completed by the police

Can I update my address when I replace my document?
No. First change your address, then order a replacement.

Can I change my name or make other changes?
Yes, but you will need go to a DMV office. See how to change your name or other information.

How will I receive my document?
We will mail it to the mailing address currently on our records or an alternate address you provide.

How long will it take?
** Allow 2 weeks for delivery of your replacement license or permit by US first-class mail.
** You can’t request delivery by Express Mail and you can’t get your photo document sooner by going to a DMV office.
** If you request a replacement driver license 41 days or more before your 21st birthday, it will say “UNDER 21.”
** a. b. If you want your new driver license sent to an alternate address, include an additional note and clearly print the alternate address.

** Don’t write the alternate address on form MV-44.
** The mailing address you write on form MV-44 must be the address that is listed on your photo document and your DMV record.
** To get a free replacement for a driver license or learner permit that was stolen or was lost due to a crime, you must get form MV-78B from a police agency.
** This form is available only from police agencies, not from the DMV.
