Legalization of Medical Documents by Public Health Institutions :

According to Article 104, topic 2 of the Constitution of the Republic of Albania and the CoMD (Council of Ministers Decision) Nr. 134, dated 7 .3. 2003,

“For the certification and legalization of Documents issued in the Republic of Albania for use outside of the country, and of foreign documents for use in the country” in accordance with article 100 of the Constitution of the Republic of Albania, article 23 of the law 8372, dated. 9.7.1998 “For application of the consular functions from diplomatic and consular representations” the Ministry of Health does the legalization of the following Health documents issued by public health institutions:
** Epicrises;
** Medical – legal reports;
** Personal Vaccination Chart;
** Qualification or attestation Certification;
** Verification of the Certification of the licensing of the private health clinics.

Required Accompanying Documents:
** The application forms issued by responsible institution at local or central level, and completed according to the requirements of the institution (obligatory name, family name, signature, stamp, protocol No.);
** Personal Vaccination Chart;
** Epicrises completed and issued by the responsible institution;
** Original Certification or notarized copy;
** Verification issued by the Responsible Institution;
** Valid personal I.D. of the person requesting the service.

Procedure Steps:
** The documents that are required to be legalized, the correct completion of the documentation is the responsibility of the interested party.
** The documents are presented at the Ministry of Health during business hours;
** The conformity of the service is checked by the responsible person;
** After the completion of the procedure the data is registered at the main register for legalizations;
** The documents are returned to the interested party within the same day.

Free of charge.

The public sector offers its services electronically through the unique governmental portal e-albania.al, in accordance with the legal acts in force in the Republic of Albania.

The National Agency for Information Society (NAIS), in accordance with the Council of Ministers Decision No. 703, dated 29.10.2014, “On the National Agency for Information Society” develops, administers and maintains the infrastructure of e- government services, the unique government portal e-Albania, the Governmental Interoperability Platform, for the delivery of e-Government services, by typology: government-government, government-citizen, and government-business.

The portal administrator, NAIS, is not responsible for possible losses stemming from the use or non-use of the services provided through the portal. In the case of electronic services, where the portal provides only the intermediary environment, the institution responsible for providing the service is responsible for the specific service.

The services provided on the portal are free of charge, excluding those services, which are provided with payment according to the legal provisions in force. The portal, in the information provided, is not responsible for the updating and accuracy of the information on the services or websites that are offered as links to it.

This is the responsibility of the respective institutions. The user information on this portal is protected in accordance with the provisions of the law on personal data protection and copyright law and other related rights.
