Application Procedure for DLSU College Admission Test (Special)
Procedure and Requirements:
Accomplish the Online Application.
1.1 Applicants will be required to indicate their chosen Academic Programs and/or Plans on the application form. The list of Undergraduate Academic Programs and Plans may be viewed or downloaded from the DLSU website.)

1.2 Applicants who will take the special examination in DLSU must accomplish and print the Online Application Form.
Proceed to the Enrollment Services Hub to submit the form together with the required documents below:
2.1 Certified Correct Secondary Scholastic Record
2.2 Two (2) Recommendation Forms from any of the following:
3rd or 4th year High School Subject Teachers
3rd or 4th year High School Class Adviser
3rd or 4th year High School Guidance Counselor
2.3 Three (3) identical passport photos
For Filipino Citizens:
A photocopy of NSO issued birth certificate (or any valid substitute)
For Non-Filipino Citizen:
ACR/ICR/ passport or photocopy of Birth Certificate
(Note: Only applications with correct and complete information will be processed.)
Pay the non-refundable Admission Processing Fee of P600.00 (Filipino and Dual Citizens) or $50.00 (non-Filipino Citizens).
Present the Official Receipt and secure Exam Permit and the Admission Kit.
All applicants must present the Exam Permits on the date of the examination.
Note:
Those who have already graduated from high school must submit a photocopy of their 4th year high school report card.
International applicants must submit a photocopy of their secondary school Transcript of Records. Upon acceptance at DLSU, applicant must secure a Certificate of Eligibility by submitting an authenticated Transcript of Records to the Record Sections of the Department of Education NCR at Misamis St., Bago Bantay, Quezon City, Philippines.
Important Reminders :
** Please keep all original hardcopies of the documentary requirements. They are necessary and form part of the requirements during confirmation of enrollment.
** Any false information given and/or fraudulent document uploaded into the online admission facility will automatically nullify the application and permanently disbar the applicant from pursuing any level of study at De La Salle University.
** Test result will be withheld for applicants with incomplete requirements and/or who failed to comply with the specifications of the documentary requirements (e.g. unclear/unreadable scanned copies). To avoid such inconvenience, inquiries may be sent to admissions AT dlsu.edu.ph with the Subject Heading: Online Application Concerns.
** Kindly take note of the reminders provided in the test permit.
** Please complete the application process early in order to avoid unnecessary inconveniences.
** Access the test result from the DLSU website at .dlsu.edu.ph on 28 April 2017 (Friday).
