Vital Records :
** The Vital Records Division processes and retains copies of all birth certificates and death certificates that occur within Davidson County.

** Certificates are issued for births after 1950, and deaths after 1966. The Vital Records Division also provides copies of medical and dental records of persons who have received treatment at a Metro Health Department clinic.
** Identification is required to obtain a certified copy of a birth or death certificate. An example of proper identification is a copy of your valid driver’s license. Please send this in along with your completed application and payment.

Birth Certificates :
** Birth certificates are available in a short and long form. Short forms are available to appropriate applicants at the price of $15 for the first copy and $15 for each additional copy.

** This form is issued from the Tennessee Computer base and is available for any birth that occurred within Tennessee from January 1, 1949 to within approximately 60 days after birth.
** Long forms provide more information and are an actual copy of the original certificate.
** This form is available for births that occurred in Metropolitan Nashville and Davidson County, Tennessee after January 1, 1966. The cost for a long form birth certificate is $15 for the first copy and $15 for each additional copy. In most cases, long forms are available within thirty days following birth.

** Come to the Office of Vital Records, located at 7201 Levander Loop, Building C (off the corner of Airport Blvd. and Hwy. 183 at the northeast corner of the Montopolis bridge on the Betty Dunkerley campus), Austin, TX, 78702.
** The office is open from 8 a.m. to 4:30 p.m. Monday through Friday. The last application is accepted until 4:15 p.m.
** Our office is usually busiest between 11 am – 2 pm. Customers should expect longer wait times during those hours.
** Current government-issued photo identification is required.
