What You Need to Provide :
** In order to request a copy of a birth certificate you must complete the form Application for Search of Birth Record Files.

** A valid government issued photo identification (ID) that must be readable and is not expired/out of date (if an ID is not provided, unreadable, or expired, the request will be returned unprocessed).

** If your request is urgent, submit it through any type of an overnight delivery service, with proof of immediate need, and a prepaid overnight delivery return envelope.
** Requests for certificates will be processed within 5 to 7 business days (for a copy of a certificate) or within 15 business days (for a correction).

As proof of immediate need include one of the following :
** A copy of a travel itinerary or plane tickets, showing the departure date within 30 days of the date on the application
** A statement from an insurance agency on its letterhead indicating coverage will be cancelled or someone won’t be added without a birth certificate
** A statement from a school on its letterhead indicating a child will not be allowed to attend school without a copy of the birth certificate

** Notice from immigration authority/agency that a birth certificate is required
** A statement on letterhead from your employer or potential employer, indicating you will not be granted employment or will have to leave employment unless a copy of the birth certificate is provided
** A statement on letterhead from a driver’s license facility indicating a birth certificate is required to obtain a driver’s license

Certified copies of birth certificates available – suitable for legal purposes :
** For $10 for the first copy of a birth certificate and $2 for each additional copy of the same certificate requested at the same time,
** you will obtain a birth certificate with the following items: child’s name; date of birth; sex; place of birth (city, county, state);
** mother/co-parent’s maiden name; mother/co-parent’s place of birth (state/country); mother/co-parent’s age; father/co-parent’s name;
** father/co-parent’s place of birth (state/country); father/co-parent’s age; file date; date issued; and state file number.
** For $15 for the first copy of a birth certificate and $2 for each additional copy of the same certificate requested at the same time,
** you will obtain a birth certificate with information collected at the time of your birth (information has varied throughout the years).
