Apply for Passport :

All passport applications should be submitted in person.

The following needs to be submitted :
** ID document (to establish applicants identity and Croatian citizenship, i.e. osobna iskaznica or putovnica)

Sponsored Links:

** Passport size photograph in color(3,5 x 4,5 cm). Important : Look straight into the camera, do not smile, with white background, face should occupy about 75-80% of the photograph surface

** Previous passport (for passport renewals)
** US visa,Green Card or US passport

** For children, applying for the first time, please include original Birth certificate (no older than six months from date of application), legalized with Apostille Seal and translated into Croatian language (if born in US).

Children over age of 12 must be present to sign the application and for giving fingerprints to be stored in the chip biometric passports.

For 1st time applicants, who have never had a Croatian passport, except the documents listed above, please submit Domovnica (Croatian citizenship certificate) or Rješenje o primitku u državljanstvo (ruling of the Minister of Interior of the Republic of Croatia that you have received Croatian citizenship), Birth certificate (not older than 6 months, legalized with Apostille Seal and translated into Croatian language).

If an applicant got married abroad and has changed his/her surname, a passport with a new name cannot be issued before the change of the name/surname has been registered in the competent Registry of Birth and the Registry of Marriage in Croatia.

Consular service comprises a consular fee of $82,00 for applicants older than 21 years and $62,00 for younger than 21. Please note we only accept personal checks or money orders (payable to Embassy of Croatia).

1.) REGISTRATION OF MARRIAGE :
For registration of marriage celebrated in the USA the following needs to be submitted :
** application form, completed and signed by both spouses, that can be found here;
** an original or notarized copy of Marriage certificate from the respective Registry office in the USA, with an attached Apostille Seal and translation to Croatian language made by certified translator;

** a valid photo-identification;
** proof of Croatian citizenship for at least one spouse.

** This consular service comprises a consular fee of USD 54,00. If you would like the document to be sent to you by post, please add USD 25,00 or an prepaid envelope for the purpose of sending the document to your address. Please note that payment is only possible by personal check or money order payable to Embassy of Croatia.

2.) REGISTRATION OF A CHILD BORN IN THE USA :
For registration of a child born in the USA the following needs to be submitted :
** downloaded and completed form “Prijava djeteta”;

** an original or certified copy of Birth certificate, sealed with Apostille seal from the respective Registry office in the USA, with an attached translation in Croatian;
** a valid photo-identification of both parents;
** proof of Croatian citizenship for at least one parent.

** This consular service is free of charge, but if you wish to obtain Domovnica (proof of citizenship) and/or Birth Certificate from Croatian Registry Office, there is a consular fee of USD 54,00. If you would like the document to be sent to you by post, please add USD 25,00 or an prepaid envelope for the purpose of sending the document to your address.

** Please note that payment is only possible by personal cheque or money order.

3.) REGISTRATION OF A DECEASED PERSON IN CROATIA :
For registration of a deceased person in Croatia the following needs to be submitted :
** downloaded and completed form “Prijava smrti”
** an original or certified copy of Death Certificate, issued by the respective Registry office in the USA, with an attached Apostille Seal and translation to Croatian language by certified translator;

** a proof of Croatian citizenship for the deceased.

** This consular service comprises a consular fee of USD 33,00. If you would like the document to be sent to you by post, please add USD 25,00 or an prepaid envelope for the purpose of sending the document to your address. Please note that payment is only possible by personal check or money order payable to Embassy of Croatia.
